【目的】これまでの調剤エラー(インシデント)に関する研究報告は、薬剤師の立場に基づいたものがほとんどであり、他職種については検討されていなかった。そこで本研究は、薬剤師と看護師の職種間に介在する調剤エラーの発生メカニズムの違いを明らかにすることを目的に行った。【方法】2006年度からの8年間で発生した入院内服薬調剤のエラーを調査対象とした。また、本研究での調剤エラー(インシデント)の患者影響レベルについては、病棟への交付に至ったエラーを「レベル0以上」、患者服用に至ったエラーを「レベル1以上」、服用後に一過性の障害が発生したエラーを「レベル2以上」と定義した。また、薬剤師と看護師のミス傾向の違いを薬名の類似性としての観点から解明するために、「規格間違い」と「薬名間違い」に含まれる文字の連続表記としての共通性(≦2文字、>2文字)に応じて、「名称類似性(-)群」と「名称類似性(+)群」のエラー2群に分類することで、両職種間のミス率の違いを2群間で比較した。【成果】「名称類似性(-)群」、「名称類似性(+)群」の薬剤師の鑑査ミス率はそれぞれ11.8%(68/576)、9.2%(43/466)であり、両群間に有意差はなかった。一方、看護師の確認ミス率はそれぞれ8.8%(6/68)、20.9%(9/43)と有意な傾向があった(p=0.0691)。また、「名称類似性(-)群」における両職種間の鑑査ミス率、確認ミス率に有意差はなかったが、「名称類似性(+)群」における両職種間のミス率には有意差があった(p<0.05)。これらのことから、看護師は薬剤師と比較して名称類似性の影響を受けやすいことが示され、これらのミス傾向の差は、名称類似性に対する両職種間の認識の違いであることが明らかとなった。本研究で得られた成果は医療安全の向上に役立つことが期待できる。さらに、本研究を含むこれまでの複数の研究成果に関する総合的な解析も行っている(12. 備考)。
