タキサン系あるいはプラチナ系抗がん剤などは,高い抗腫瘍効果が得られる反面,その副作用として末梢神経障害が高頻度発現することが知られており,患者のQOLを著しく低下させる。本研究では漢方薬である牛車腎気丸の抗がん剤起因性末梢神経障害に対する臨床効果を検討するとともに、効果と血流増加因子(CGRP、VIPなど)の血液中濃度変化との関連を明らかにすることを目的とした。はじめに産科婦人科および腫瘍内科でタキサン系あるいはプラチナ系抗がん剤を含む抗がん剤治療施行患者40名において末梢神経障害の発現と薬剤の使用状況について後向きに解析した。その結果、末梢神経障害は全患者中75%以上に高発現していた。またその治療薬としてはビタミン剤と牛車腎気丸や芍薬甘草湯が主として使用されていたが、改善効果は牛車腎気丸において最も認められた。次に、健常人における検討では牛車腎気丸を単回投与した場合、血流量の増加とともに血流調節因子である神経ペプチドの血中濃度が上昇することが明らかとなった。上記の疫学的調査と健常人における検討により、牛車腎気丸は抗がん剤起因性の末梢神経障害に対して高い改善効果を有し、さらにその薬理作用として一部神経ペプチドを介している可能性が考えられた。これは、未だ有効な治療薬がない抗がん剤起因性の神経障害に対して本剤が有効な薬剤となりうる可能性を示す重要なデータであると考えられる。また血液中の神経ペプチド濃度の測定が、牛車腎気丸の薬理効果を明らかにするとともに、複合的な作用を有する本剤の薬理効果評価法として有用である可能性を示している。以上の結果をもとに現在は患者の集積と神経ペプチド濃度の測定を進めている。
