手指の機能障害患者においては、一般に義歯を装着することは可能だが、外すことが困難である場合が多い。そこで、本研究では、(1)「義歯を容易に取り外すことのみを目的とした装置」と(2)「義歯の取付けと取り外し(着脱)を容易にする装置」、とに分けて作成を試みた。方法としては、義歯の取り外しが上手く出来ない患青に、義歯を装着した状態で印象採得を行い、義歯装着時の口腔内と同じ状態の模型を作製し、その模型上で、義歯を簡便に着脱出来る装置の開発を行った。(1) 義歯を容易に取り外すことのみを目的とした装置の開発模型上の義歯に数箇所の取り外し用フックを付け、フックは患者の口腔内を傷付けないように、歯科用のボールクラスプを使用した。それぞれのフックを牽引し、どの程度の力で義歯が模型上から外れるかを計測し、フックを付ける最適な場所を選択し、ついで、そのフックに簡単に力が加えられるように歯科用コバルトクロム製の鉤線を曲げ、その鉤線を患者の手指の大きさに調整した。患者の手指に付けた鉤線と義歯に付けたフック〓より、義歯を外せるような装置を作成した。(2) 義歯の着脱を容易にする装置の開発義歯の形態に合わせ、歯科用アクリルレジンを用いてその義歯を覆う形のもの(部品A)を作製した。部品Aには患者が把持出来る形態の柄を付与し、義歯を覆う部分には、歯科用ボールクラスプで作製したフックを数箇所付け、それに対応する義歯の部分にフックが掛かるレール状の溝を作った。模型上で最適なフックの数、強さを検討した後、義歯を覆つたレジンの上面に対側の噛み合せの圧痕を付け、患者が噛み締めることにより義歯が装着出来る装置を作成した。また、取り外し時には装置を義歯の上に乗せ、噛み締めることにより、装置のフックに義歯が引っかかり外せるように調整した。
