项目摘要

研究対象は、東京大学医学部附属病院・顎口腔外科・歯科矯正歯科を受診した患者の中で、開口制限を有し、通常の既製トレーあるいは個人トレーの使用では印象採得が困難な症例とした。分割トレーの場合、各パーツは、(1)印象採得時に口腔内で組み合わされ、(2)口腔外に取り出す際に一旦分割され、(3)石膏注入時に再度組み合わされるが、従来報告されていた方法では、(1)～(3)の各ステップにおいて、分割トレーの各パーツの位置関係が狂ってしまい、最終的に得られる石膏模型の形状変化・精度低下が起こる可能性がある。また最終印象の質が、術者の経験・技量にかなり左右される面もあることは否定できない。そこで、本研究では、これらの欠点を解消し、開口制限を有する患者の印象採得を容易にする分割トレーを開発するために以下の点を検討した。I.従来から使われている歯科技工用材料の中で、物性・適合性・口腔内での操作性といった面で、分割トレー作成に最適と思われる材料を選択した。II.印象採得に最適な分割形状(各パーツの形状)を検討した。III.分割状態で口腔内から取り出されたトレーを、石膏注入時に再び口腔外で印象採得時の状態に組み合わせる時に再現性良く組み合わせることが出来るような補助装置-分割トレー安定化装置-の開発を行った。なお、本研究では特にIIIの項目に力点を置き、分割トレー安定化装置の開発にあたっては、マグネットの使用、パーツを組み合わせる際のメルクマールとなる凹凸の付与、パーツを安定化するために使うバインダーの開発など、再現性良くパーツ同士を組み合わせることが出来る方法を検討した。これらの点について、患者の感想、歯科医師の意見を参考にしながら、分割トレーの改良を行い、従来法に比べて良好な結果を得ることが出来た。
