【研究目的】:同種造血細胞移植治療を行う上で、移植後GVHD (graft versus host disease)の発症をいかに早期に診断し治療するかは、患者の生命予後を左右する上で極めて重要な問題である。移植後GVHDは、移植後2～3週頃に好発する疾患で、その診断は皮疹、肝機能障害、消化器症状等の臨床症状や病理診断により行われているのが現状である。そして、典型的な症状が出現する場合にはその診断は比較的容易であるが、非典型例の場合では診断に苦慮する場合が多く、その場合には死に至るケースも少なくない。そこで、本研究は、同種造血細胞移植患者においてCD3陽性、CD4陽性とともに、活性化Tリンパ球に特異的なCD134陽性細胞を測定し、比較検討することにより、移植後GVHDの早期診断を行うことが可能か否かを検討する。本研究により、ヒトにおいてもCD134とGVHDとの関連性が証明されれば、将来的にGVHD予防薬の適宜使用や投薬量の減量による患者QOLの向上につながるものと考えられる。【研究方法】:当院では年間10例前後の造血細胞移植が施行されているので、これらの患者を対象とし、フローサイトメーターにて対象患者の末梢血のTリンパ球(CD3陽性/CD4陽性)と活性化Tリンパ球(CD3陽性/CD4陽性/CD134陽性)を経時的に計数し、その比(Tリンパ球活性化率:%Activated T Cell ; %ATC)=活性化Tリンパ球/Tリンパ球を求め、臨床所見と対比する。【研究成果】:AML(M4)非血縁者間骨髄移植患者10E001のTリンパ球活性化率は、移植前6.2%、移植後(day20～53)7.6±2.4%(n=9)、健常人4.2±1.9%(n=9)であり、観察期間中患者はGVHDを発症しなかった。移植前後のTリンパ球活性化率に差を認めず、健常人との比較で差があることが示唆されたが、今後GVHD発症患者、未発症患者両面で統計解析に耐えうるデータの集積が必要である。

[Objective]: The development of allogeneic hematopoietic cell transplantation therapy, early diagnosis and treatment of post-transplant GVHD (graft versus host disease), and the survival of patients are important issues. GVHD after transplantation is usually diagnosed as a disease 2 - 3 weeks after transplantation, including rash, liver dysfunction, digestive symptoms, and pathological diagnosis. For typical cases, diagnosis is easier than for atypical cases, diagnosis is more difficult, and diagnosis is less difficult. In this study, CD3-positive, CD4-positive, activated T cells and CD134-positive cells specific to allogeneic hematopoietic cell transplantation patients were measured, compared, and evaluated for possible early diagnosis of GVHD after transplantation. This study aims to demonstrate the relationship between CD134 and GVHD, and to examine the appropriate use of GVHD prophylaxis and dose reduction in patients with QOL. [Methods]: Ten patients before and after hematopoietic cell transplantation were enrolled in this study during the same period. The number and ratio of Activated T cells (CD3 positive/CD4 positive) and activated T cells (CD3 positive/CD4 positive/CD134 positive) in peripheral blood of these patients were calculated and compared with clinical findings. [Results]: The activation rate of T lymphocytes in AML(M4) patients with non-hematologic bone marrow transplantation was 6.2% before transplantation, 7.6±2.4%(n=9) after transplantation (day20 - 53), and 4.2±1.9%(n=9) in healthy subjects. The difference of T lymphocyte activation rate before and after transplantation, the difference between healthy people and healthy people, and the statistical analysis of T lymphocyte activation rate in patients with and without GVHD in the future are necessary.
