【研究目的】シスプラチン(CDDP)は優れた抗腫瘍効果をもつ反面、副作用の発現率が高く、腎障害もその一つに挙げられる。本邦では、CDDPの腎障害を軽減する目的で10時間以上におよぶ大量の水分負荷を行うことが推奨されている。海外においては、より少量の水分負荷(ショートハイドレーション:short hydration)で腎毒性管理が可能であることが報告され、外来にて投与されている。本邦においても外来でのCDDP投与が可能となれば、患者のQOL維持や医療費の負担軽減が期待できる。近年、優れた制吐剤の開発によりhydrationの削減に関する検討が可能となり、肺癌患者でのCDDPを含む化学療法のshort hydrationレジメンを検討した。このshort hydrationレジメンによる治療を受けた患者と従来のレジメンによる治療を受けた患者の腎機能を比較し、肺癌患者におけるshort hydrationでの安全性および忍容性について評価を行った。【研究方法】対象者は、CDDP 60mg/m^2以上を投与された肺癌患者とした。2007年9月から2008年12月に従来のレジメンによる治療をされた患者をstandard群、2009年1月から2010年4月にshort hydrationレジメン(Day 1における輸液量:約2/3、投与時間:約1/2)によって治療された患者をshort群とした。【研究成果】対象者は、両群ともに28症例であった。short群において1例(3.6%)のみが腎機能障害のために治療が中止された。1コース目において腎機能障害がgrade 1 (CTCAE v3.0)であった患者は、standard群9例、short群3例であり、grade 2以上は見られなかった。血清Creatinine値の150%以上の上昇かつ施設基準の上限を超えた患者数はstandard群で3例、short群は1例であった。尿素窒素値で10mg/dL以上の上昇かつ施設基準の上限を超えた患者数はstandard群で6例、short群は6例であった。腎障害の発症頻度は、両群間で有意な差はなかった。また、short群の13例(46.4%)については2コース目に外来治療へ移行することができた。本研究の成果として、肺癌患者におけるCDDPを含む併用化学療法でのshort hydrationは忍容可能であることが示唆された。
