项目摘要

同種造血幹細胞移植後のドナー細胞とレシピエント細胞の比率を評価する、キムリズム解析には標準法がなく、院内検査では4～6種類のSTR領域毎にシングルPCRを行って、識別領域を決定している。現行法と同等の感度や再現性を有し、解析時間やコスト面でも良好なマルチプレックスPCRの構築と、移植後モニタリングのための検査材料の評価を目的とした。1.ドナー・レシピエント識別率の高いSTR領域の選別と組み合わせの検討移植例41組での7種類の各STR領域のドナー・レシピエント識別率は、約24%から44%であった。VWF、SE33、D18S51の組み合わせ(multiplex STR-PCR-A)とD5S818、FGAの組み合わせ(multiplex STR-PCR-B)によるマルチプレックスPCRを行った結果、識別率を92.7%まで上げることが可能であり、シングルPCRと同等の感度も得ることができた。染色体座位の異なる2種類以上のSTR領域を組み合わせることで、領域間での再現性を確認出来ることに加え、付加的染色体異常が認められる再発時でも正確なキメリズム検査が可能となり臨床的有用性が高まると考えられる。2.移植後キメリズム検査に使用する検査材料の適正評価移植施行70例における、移植後約30日のキメリズムと完全キメラ移行期間についての比較から、全血、単核球、CD3陽性細胞の3種類の検査材料の評価を行った。単核球分画は分離が容易であることに加え、T細胞だけでなく他の細胞系列を含むことから、完全キメラへの移行期間の長期化や生着不全など、移植後の状態をより正確に反映していた。以上により、操作効率およびドナー・レシピエント識別率を向上させたマルチプレックスPCR法の確立と、単核球分画の臨床的有用性を明らかにすることができた。

After allogeneic hematopoietic stem cell transplantation, the ratio of allogeneic stem cells to allogeneic stem cells was evaluated, and standard methods were used to analyze allogeneic stem cells. In hospital, 4~6 STR domains were examined by PCR, and identification domains were determined. The sensitivity and reproducibility of the current method are different, the analysis time is different, and the PCR construction is good. The evaluation of the test material after transplantation is different. 1. The recognition rate of 7 STR types in 41 STR groups ranged from 24% to 44%. VWF, SE33, D18S51 and D5S818 were used for multiplex STR-PCR-A and multiplex STR-PCR-B respectively. Chromosome loci with more than 2 types of STR domains are grouped together, and reproducibility between domains is confirmed. Chromosome abnormalities added and added are recognized and reproduced. Correct STR detection is possible. Clinical usefulness is high. 2. Appropriate evaluation of the test materials used in the post-transplant examination was performed in 70 cases of transplantation, a comparison between the post-transplant examination about 30 days after transplantation and the complete post-transplant period, and an evaluation of three types of test materials: whole blood, mononuclear cells, and CD3-positive cells. It is easy to separate T cells from each other, and T cells can be separated from each other. It is easy to reflect the status of T cells after transplantation. The establishment of PCR and the clinical usefulness of PCR analysis are discussed in detail.
