【目的】当院では直腸炎型または遠位型潰瘍性大腸炎患者に対する局所療法として、市販されている注腸剤と同じくメサラジンとして1gを含有する坐剤を作製し使用している。これまでにメサラジン1g含有坐剤を使用した患者の血便回数の減少や、排便時の切迫感の改善などに効果がみられている。しかし注腸剤と比較して直腸部に限局した効果を期待できる坐剤では、より少ない用量で効果が得られる可能性がある。【方法】メサラジン1g含有坐剤を使用し緩解状態が得られている患者に対して500mg含有坐剤へ切り替えを行い、その緩解維持効果を検討した。投与方法は1日1回、就寝前に1個使用することとした。評価項目は1日の便回数、血便回数、切迫感評価、有害事象、坐剤の使用感とし、500mg含有坐剤に切り替える前と比較した。【結果】500mg含有坐剤へ切り替え後も状態の悪化がなく、また1g含有坐剤と比較してその大きさがおよそ半分であることより使用しやすいとの意見が得られた。潰瘍性大腸炎は緩解と再燃を繰り返しやすい疾患のため、薬剤を長期的に使用することとなる。より少ない用量で大きさも小さい500mg含有坐剤はコンプライアンスの向上に貢献する可能性が示唆された。
