【目的】喘息や慢性閉塞性肺疾患は、日常生活を脅かし生命の危険をも考慮すべき疾患であり、吸入薬による治療管理が重要とされている。吸入薬には、手技の適否が治療効果に影響するという問題点がある。近年、各製薬企業から様々なデバイスが発売されているが、薬剤毎に形状や使い方が異なるため、患者のみならず薬剤師の間でも混乱を招いている現状がある。外来での継続的な吸入指導実施のために、薬剤師による定期的な手技確認、患者に応じた適切なデバイスへの変更を行い、病薬連携を推進するための検討を行った。【方法】処方箋応需薬局の薬剤師を対象として、6月に呼吸器内科の医師と共に吸入指導の統一を図る説明会を開催した。各デバイス毎に操作手順を記載した「吸入手技評価票」を作成し、評価票を参照して指導を行うことで吸入指導に不慣れな薬剤師でも確実に指導できるように工夫した。さらに3月にデバイスを適正に判断するため、吸気流速を測定できる「インチェック〓」を用いた指導の説明会を開催した。指導内容は「吸入手技評価票」に記載後FAXで報告し、院内の電子カルテに結果をフィードバックすることで、情報の共有を図った。【結果】説明会には、近隣の薬局から約30名が参加した。参加者に行ったアンケートで、吸入速度の指導が最も難しいと感じていることが判明したが、吸気流速の測定を取り入れたことによって、適切な評価が可能になった。「吸入手技評価票」の内容についてはデバイス毎に手順がわかり、確実に指導ができる、と全ての薬剤師から評価された。本取り組み前後で、フィードバック件数は8.3件/月から15.7件/月に増加し、特に3月の説明会後は30件/月を超えた。また、初回の指導のみならず、2回目以降の指導報告もみられるなど、継続的な吸入指導が行われるようになり、評価票を用いた病薬連携は、適切な吸入指導に大きく貢献している。
