我が国では麻疹ゼロを目指しているが未だ根絶されておらず、予防接種の理解が求められている。2012年は2008年から5年間の時限措置である高校3年生を対象とした第4期予防接種が最終年度を迎え、S高校の2008年から4年間の第4期予防接種歴と感染症に関する教育との関連第1期予防接種歴と第4期予防接種歴について解析することを目的とした。2008年および2010年にG県のS高校の生徒1,155人を調査対象として予防接種歴調査を行った。2008年から4年間の第4期接種該当者は915人であり、第4期予防接種歴について調査した。また、感染症に関する教育を行い、教育前後にアンケート調査を実施し、第4期予防接種歴と感染症に関する教育実施前後の相関、第1期接種歴と第4期接種歴の相関について分析し、Pearsonのカイ2乗検定を用い検定を行った。S高校の生徒1,155人の第1期接種歴は、1回接種者1,018人(88.1%)、不明者9人(0.8%)、未接種者128人(11.1%)であった。第4期接種者は2012年3月までの対象者915人中563人(50.6%)であった。その中で、第1期接種において1回接種者の割合は90.1%(507/563人)、不明者0.2%(1/563人)、第1期未接種者97%(55/563人)であった。第1期未接種者及び不明者は第4期接種率が低く有意な差がみられた(P<0.05)。本研究における高校3年生の第4期接種率は、2008年度S高校5a2、G県8a8、全国77.3%、2009年度68.7、79.5、77.0%、2010年度91.0、82.9、78.9%、2011年度85.1、76.8、62.5%であり、S高校は、2010年、2011年は県、全国平均よりも高い接種率であった。「感染と免疫の基礎知識1を基に作成した5つの質問について、教育効果を調べたところ、第4期接種群では教育後に「知っている」と回答した割合は教育前において増加し、すべての項目で有意差がみられた(P<0.0001)。また、第4期接種率向上に向けた取り組みを通して、第4期接種の接種者と未接種者では、接種者においては第1期接種においても接種しており、未接種者においては第1期接種においても未接種である人の割合が高く有意差がみられた。
