体外式膜型人工肺施行患者におけるカルバペネム系抗菌薬の薬物動態の検討

本研究は、体外式膜型人工肺(ECMO)施行患者のカルバペネム系抗菌薬の血中濃度を測定することにより、カルバペネム系抗菌薬の血中動態と効果との関係性を評価することを目的とし、以下の研究実施計画をもって申請された。まず、ECMO施行患者の内、カルバペネム系抗菌薬を投与する患者を対象とし、抗菌薬投与前(トラフ値)、投与後1、3、6時間後に採血を行う。血中濃度はHPLCを用いて測定する。血漿400μLを限外濾過し、20μLを逆相C18カラムで分離し、300nmの吸光度を測定する。患者背景として、性別、年齢、体重、身長、血清クレアチニン値、ECMO施行の理由、感染症起因菌、最少発育阻止濃度を調査する。抗菌薬の治療効果はC反応性タンパク質、患者予後はECMO施行からの生存日数により評価する。患者予後に及ぼす薬物動態以外の因子についてはロジスティック回帰分析により評価する。現在、本研究はサンプル収集を継続している。血中濃度測定法は超高圧液体クロマトグラフィー紫外吸光検出法を用いたメロペネム測定系を構築しており、サンプルサイズが揃い次第解析を行い、得られた結果については学会発表や論文発表の形で公表する。また、本研究についてミーティングを重ねた結果、カルバペネム系抗菌薬以外にもニューキノロン系抗菌薬、セフェム系抗菌薬、βラクタマーゼ含有ペニシリン系抗菌薬の中にも検討が必要な薬剤があるとの見解があり、体外循環の種類についてもECMOだけでなく、血漿交換、ビリルビン吸着もしくはエンドトキシン吸着についても検討が必要である。抗菌薬の種類および体外循環の種類に関しては改めて評価を進めている。

The purpose of this study is to evaluate the relationship between the blood concentration of an antimicrobial agent and the blood dynamics of an antimicrobial agent in patients undergoing an extracorporeal membrane artificial lung (ECMO). Blood samples were collected at 1, 3, and 6 hours before, after, and after administration of antimicrobial agents in patients undergoing ECMO. Blood concentrations were determined by HPLC. 400μL of plasma was filtered out, 20μL was separated by reverse-phase C18 separation, and absorbance at 300nm was determined. Patient background, gender, age, weight, length, serum creatinine, reason for ECMO, cause of infection, and minimum reproductive inhibition concentration were investigated. Antibacterial agents are effective in determining the quality of treatment and the number of days patients survive after ECMO. Factors other than patient prognosis and biodynamics may be used to assess patient outcomes. Now, this study is about the collection of samples. The blood concentration determination method uses ultra-high pressure liquid detection method to construct the determination system, and obtain the results of the analysis. In this study, we reviewed the results of the study, including the identification of antimicrobial agents, the identification of essential agents, the identification of ECMO agents, plasma exchange, and the identification of CPB agents. It is necessary to examine the contents of the application. The type of antibacterial agent and the type of extracorporeal circulation are related to the change of evaluation.
