研究目的 : 身体運動が制限される時期における筋力低下は日常生活動作に影響を与え、不動化による筋力低下により大脳皮質の活性化領域が変化することが報告されている。運動の実施なしに課題を認知的にリハーサルするメンタルプラクティスおよび訓練実施の2週間介入による手指の巧緻性評価および大脳皮質の局所脳血流の変化から手術後早期や傷病急性期に身体運動が制限された状況下で有効な手指巧緻動作トレーニングの確立を検討した。研究方法 : 右手利きの健常者45名を3群に分け、コントロール群(15名)は何ら実施せず、メンタルプラクティス群(15名)は机に向かった椅子坐位姿勢で、右手で持った箸で小球を摘まみ一方の皿から他方の皿に移す動作をイメージさせた。運動イメージは1日に2分間イメージ持続を5回、イメージ間に30秒の休息を取り入れ、運動イメージの補完のためイメージと同様の動作の動画を2分間視聴した。訓練実施群(15名)は、机に向かった椅子坐位姿勢にて右手で持った箸で小球を摘まみ皿から皿に移す動作を1日に2分間を5回実施、実施間に30秒の休息を取り入れた。2群とも週5日、2週間実施した。介入効果を検討するため、Box and Block Testを用いた手指の巧緻性評価(3回測定)と同時に近赤外光イメージング装置(0MM-3000、島津製作所)を用いて両側の一次感覚運動野における局所脳血流の変化を3群とも実施前、2週間実施後に測定した。研究成果 : Box and Block Testはメンタルプラクティス群と訓練実施群において有意な増加(p<0.01)を認め、局所脳血流は3群とも有意な変化を認めなかった。今回、箸動作のメンタルプラクティスにおいて手指の巧緻性の向上を認めたが、一次感覚運動野における活性化の変化を伴わないと考えられた。大脳皮質の活性化を増加させるまでに至らないがパフォーマンス向上の面から、身体運動制限の状況下でメンタルプラクティスによる手指巧緻動作トレーニングの有効な可能性が示唆された。
