【研究目的】今回の研究では、舌痛症患者と特発性歯痛患者のQOLを含む特質を調べ比較し、綿運動という軽負荷運動、開口リラクセーション指導による自立訓練を行う事での経過を調べることを目的とする。【研究方法】愛知医科大学痛みセンターを受診した舌痛症患者と特発性歯痛患者で、調査研究参加に同意を得た方に対して、質問紙検査、唾液量検査(サクソンテスト)、心拍変動測定を初回と開口リラクセーション指導による自立訓練3か月後の2回実施し、データを収集し、検討する。【研究成果】舌痛症患者6名(平均年齢64.1歳、男 : 1、女 : 5)特発性歯痛患者11名(平均年齢47.6歳、男 : 1、女 : 10)の初回データを収集することができた。2回目のデータは、期間内に収集できなかった。初回データを舌痛症群と特発性歯痛群とに分け、それぞれの平均値を比較してみると、舌痛症患者の痛みによる日常動作への影響(PDAS)は、特発性歯痛患者より大きく、それ以外の疼痛強度(NRS)、HADS不安尺度、HADSうつ尺度、痛みの破局化思考(PCS)は、舌痛症患者より特発性歯痛患者のほうが高かった。唾液検査(サクソンテスト)においては、予想外に特発性歯痛患者の8/10名が2. Ogより低かった。舌痛症患者は、全員2.0gより低かった。軽負荷の綿運動(約5分間)を行う前後で行った心拍変動の計測より得られたHFの平均値は介入前と後でともに両患者群間に有意差が認められた。しかし、各患者群における軽負荷前後の有意差は認められなかった。より正確な統計的評価をするためにさらにデータ数を増やしていく必要があると考えられる。また、継続して自立訓練3か月後のデータを収集し鎮痛効果に関する検討をしたい。なお、今回の研究結果については、平成27年日本口腔顔面痛学会での発表を予定している。

[Objective] This study aims to compare QOL adjustment between patients with glossodynia and patients with idiopathic dental pain. It aims to compare QOL adjustment between patients with glossodynia and patients with idiopathic dental pain. [Methods] Aichi Medical University Patients with glossodynia and idiopathic dental pain were examined for participation in the study, and consent was obtained. Questions were examined. Saliva volume was examined. Cardiac activity was measured. Self-reliance training was conducted three months later. [Results] Six patients with glossodynia (mean age 64.1 years, male: 1, female: 5) and 11 patients with idiopathic dental pain (mean age 47.6 years, male: 1, female: 10) had initial recovery. 2. The time limit for the collection of items is fixed. The average values of initial and idiopathic dental pain groups were higher than those of initial and idiopathic dental pain groups, and the daily movement effect of pain in patients with glossodynia (PDAS) was higher than those of idiopathic dental pain groups, and the pain intensity in patients with idiopathic dental pain (NRS), HADS disturbance scale, HADS disturbance scale, and pain fragmentation thinking (PCS) were higher than those of idiopathic dental pain groups. 8/10 patients with idiopathic dental pain were diagnosed with saliva examination. Og. Tongue pain patients, all 2.0g. The measurement of heart beat before and after the intervention was performed. There is a significant difference between patients before and after the load. The number of correct statistics is increasing. After three months of self-reliance training, the results of the study were discussed. The results of this study are expected to be published in the Japanese Society for Oral and Facial Pain in 2007.
