本年度は抗SS-B/La抗体陽性例のHLAクラスII遺伝子および同遺伝子と臨床像との関連を追求した。【方法】(1)対象:慶應義塾大学病院内科を受診したシェ-グレン症候群54例を対象とした。これらの患者より得た血清、末梢血白血球を試料とした。(2)HLA Class II(HLR-DE B1,DQA1,DQB1)遺伝子タイピング:患者HLAタイピングには,PCR-RELP法を用いた。(3)自己抗体の解析:自己抗体はrecombinant SS-B/La,SS-A/Ro蛋白を用いたELISA法および^<32>P-燐酸または^<33>S-Met標識細胞成分を用いた免疫沈降法で行った。(4)唾液分泌機能の評価:^<99m>Tc-O4を用いた機能的RIシンチグラフィーにより調べた。(5)HLAタイピングの臨床的意義の解析:シェ-グレン患者及び対照疾患患者を、HLA Class II遺伝子、抗SS-B/La抗体、抗SS-A/Ro抗体、などの自己抗体、により層別化し、唾液腺機能の重症度を比較検討した。【結果】(1)対象シェ-グレン症候群患者を自己抗体の特異性により層別化し、HLA Class II遺伝子との関連を検討した。DR B1の分析では、抗SS-B/La抗体陽性例(La>Ro)でDR B1^*1101が29%と健常人の2%に比べ、有意に高頻度であった。DQ A1では、抗SS-B/La抗体陽性例(La>Ro、Ro>Laとも)でDQ A1^*0501が高頻度であった(P<0.05)。DQ B1では、DQ B1^*0301が抗SS-B/La抗体陽性例(Ro>La)で64%と高頻度であった。(2)抗SS-A/Ro抗体,抗SS-B/La抗体が検出されない例33%,抗SS-A/Ro抗体単独陽性例88%,抗SS-B/La抗体陽性例(La>Ro)36%,抗SS-B/La抗体陽性例(Ro>La)64%に高度機能障害を認めた。しかし、唾液腺機能とHLA Class II遺伝子との関連は明らかでなかった。【結語】シェ-グレン症候群における抗SS-B/La抗体、抗SS-A/Ro抗体産生とHLAクラスII遺伝子との関連が明らかとなったが、かかる遺伝子とシェ-グレン症候群の臨床像との関連、さらに同抗体産生に関与するT細胞機能を追究することが重要な課題である。

This year, we are looking for the relationship between anti-SSB/La antibody positive cases and HLA diagnosis II. [Methods](1) Subjects: 54 patients with Keio Keio syndrome were diagnosed in the Department of Internal Medicine, Keio University Hospital. The patient's serum and peripheral white blood cell samples were obtained. (2)HLA Class II(HLR-DE B1,DQA1,DQB1) genetic information: HLA classification,PCR-RELP method. (3)Analysis of antibodies: recombinant SS-B/La,SS-A/Ro proteins were identified by ELISA and <32>P-phosphate and <33>S-Met labeling cell components were identified by immunoprecipitation. (4)Salivary secretion function evaluation:^<99m>Tc-O4 function RI function (5) Analysis of clinical significance of HLA classification: comparison of HLA Class II gene expression, anti-SS-B/La antibody, anti-SS-A/Ro antibody, anti-self antibody, classification, severity of salivary gland function. [Results](1) To investigate the association between HLA Class II gene and specificity of antibodies in patients with SIRS. DR B1 analysis showed that anti-SSB/La antibody positive cases (La&gt;Ro) were 29% higher than those of healthy people and 2% higher than those of intentional cases. DQ A1^*0501 high frequency of anti-SS-B/La antibody positive cases (La&gt;Ro, Ro&gt;La) DQ A1^*0501 high frequency (P&lt;0.05). DQ B1, DQ B1^*0301 anti-SS-B/La antibody positive cases (Ro&gt;La) 64% high frequency. (2)Anti-SS-A/Ro antibody, anti-SS-B/La antibody were detected in 33% of the cases, anti-SS-A/Ro antibody positive in 88%, anti-SS-B/La antibody positive in 36%(La&gt;Ro), anti-SS-B/La antibody positive in 64%(Ro&gt;La). The relationship between salivary gland function and HLA Class II genes [Conclusion] The relationship between anti-SS-B/La antibody, anti-SS-A/Ro antibody production and HLA class II gene expression in patients with SCDS is an important topic.
