本年度は,抗シグナル認識粒子(Signal Recognition Particle:SRP)抗体の臨床的意義,免疫学的特徴を追究した。【方法】1)慶大および関連施設の膠原病患者2020例を対象とした。2)自己抗体は,LernerとSteitzに順じ,免疫沈降法で分析した。3)HLAクラスII遺伝子はPCR-RFLP法で同定した。4)抗SRP抗体陽性例の臨床免疫学的特徴を検討した。【結果】1)同抗体陽性14例(多発性筋炎:11,皮膚筋炎:1,重複症候群:2例)が見出された。2)5例はステロイド療法抵抗性筋炎で,免疫抑制剤の併用を要した。3)筋組織を詳細に検討し得た同抗体陽性全7例に炎症性細胞浸潤がなく,4例にtype1 fiber優位を認めた。4)抗SRP全14血清がSRP-72kD,9kDサブユニット蛋白と反応したが、3血清は主要抗原蛋白とされる54kD蛋白を認識せず,症例により各蛋白成分との反応性が異なっていた。5)HLA DR8を36%(DRB1^*0802を21%)に認め,高頻度であった。【考察・結語】1)抗SRP抗体陽性例はステロイド抵抗性筋炎と密接に関連し,同抗体の測定は,診断,病型分類,治療法の選択など臨床的に有用と考えられた。2)筋組織の検討では,炎症性細胞浸潤がなく,type1 fiber優位が特徴的で,同抗体陽性例のステロイド不応性筋炎を追究する上で重要な知見と思われた。3)抗SRP抗体の抗原認識様式は多様で,今後さらに詳細な分析が必要である。

This year, the clinical significance and immunological characteristics of anti-SRP antibodies were investigated. [Methods] 1) A total of 2020 patients with collagenous diseases were enrolled in this study. 2)Self-antibody,Lerner and Steitz were analyzed by immunosedimentation method. 3)HLA II gene was identified by PCR-RFLP. 4)Clinical immunologic features of anti-SRP antibody positive cases were discussed. [Results] 1) 14 cases were positive for the same antibody (11 cases of polymyositis, 1 case of cutaneous myositis, 2 cases of duplication syndrome). 2)5 For example, in the treatment of resistant tendonitis, immunosuppressive agents are used in combination. 3)All 7 cases had inflammatory cell infiltration and 4 cases had type1 fiber. 4)Anti-SRP-14 serum contains SRP-72kD,9kD protein and anti-SRP-14 protein. Anti-SRP-14 serum contains SRP-72 kD, 9 kD protein and anti-SRP-14 protein. 5)HLA DR8 36%(DRB1^*0802 21%) is recognized, high frequency. 1) Anti-SRP antibody positive cases are closely related to the detection of anti-SRP antibody, diagnosis, classification of disease type, selection of treatment method, and clinical usefulness. 2)Tendon tissue examination, inflammatory cell infiltration,type1 fiber predisposition, and antibody-positive cases of inflammatory tendon inflammation are important findings. 3)Antigen recognition patterns of anti-SRP antibodies are diverse, and detailed analysis is necessary in the future.
