项目摘要

基底膜に対する血中自己抗体を有する自己免疫性水疱性疾患として、水疱性類天疱瘡・妊娠性疱疹・瘢痕性類天疱瘡・後天性表皮水疱症・線状IgA水疱症などがあるが、これらは様々な臨床症状を呈し、かつ血中自己抗体が非常に低濃度で存在することも多い。これらのうち前3者は基底膜へのヘミデスモソームに局在する膜貫通性蛋白のひとつである180kD類天疱瘡抗原に対する血中自己抗体が病因に関与し、このうち細胞外に存在するNC160a領域がこれらの水疱症の共通抗原とされている。今回我々は、この領域に対応するcDNAを用いて大腸菌による融合蛋白を作成し、これらの水疱性疾患患者血清との反応性を免疫ブロット法にて調べ、その特異性、さらに病因への関与に関して検討を行った。その結果、類天疱瘡患者において、直接蛍光抗体法・間接蛍光抗体法ともに陽性の例では、ほぼ全例にNC16a陽性所見が認められた。特に、1MNaClで表皮を分離した検体を用いた間接蛍光抗体法においても弱陽性所見しか得られなかった症例や、寛解期の症例、また通常のヒト表皮抽出物を基質とした免疫ブロット法では230kD類天疱瘡抗原にのみ陽性で、180kD類天疱瘡抗原には陰性の症例でも陽性所見が認められた。妊娠性疱疹においては、症例数は少ないものの全例で陽性が認められ、臨床診断の確認に非常に有用であった。後天性表皮水疱症・天疱瘡・正常コントロールではすべて陰性で感度・特異度ともに高かった。以上のことから、類天疱瘡・妊娠性疱疹に対する簡便でかつ迅速な検査法が確立され、特に日常診療においてしばしば診断に苦慮する低抗体価、非定型例の診断において非常に有用と思われた。

The basement membrane is associated with self-antibodies in the blood. Immunological blistering disorders, blistering pemphigoid, herpes gravidarum, cicatricial pemphigoid, acquired epidermal blistering disease, linear IgA blistering disease, clinical symptoms, and very low levels of self-antibodies in the blood are present. The first three of them are related to the pathogenesis of basal membrane disease, membrane penetrating protein and 180kD pemphigoid antigen. The common antigen of vesicular disease is related to the extracellular existence of NC160a domain. In this paper, we have studied the relationship between the specificity, etiology and immune response of patients with vesicular diseases by using cDNA fusion protein in Escherichia coli. The results showed that pemphigoid patients were positive by direct and indirect light antibody methods, and all cases were positive by NC16a. In particular, 1 M NaCl was used to isolate epidermal cells, and indirect light antibody method was used to detect weak positive results. In addition, in general, epidermal extract was used to detect positive results for 230kD pemphigoid antigen and negative results for 180kD pemphigoid antigen. Herpes gravidarum is very useful in identifying and confirming the clinical diagnosis. Acquired epidermal blistering disease, pemphigus, and normal skin disease are both negative and have high sensitivity and specificity. These simple, rapid tests for pemphigoid and herpes gravidarum are very useful for establishing, especially for routine diagnosis, low antibody levels, and diagnosis of atypical cases.
