高齢の長期入院透析患者が抱える精神心理状態の解析のために、面接法と質問紙法による心理テストを用いて様々な角度から検討を行った。本年度は、高齢透析患者全般にわたりProfile of Mood Status,Dealing with Illness,Marlone-Crowne Social Desirabilityといった質問紙法と面接法を用いて解析を行った。Profile of Mood Statusに現れる『抑鬱・落ち込み』、『活気』、『怒り・敵意』、『疲労』、『緊張・不安』、『混乱』といった項目の中で、高齢透析患者においては、『怒り・敵意』、『緊張・不安』の項目が、若年透析患者よりも低い傾向が認められた。感情面においては、高齢透析患者は、透析という強いストレス下でも安定した状態を保っていることが判明した。Dealing with Illnessの解析からは、透析というストレス下でも高齢透析患者は、積極的に行動しようとする面をもつが、一方で受け身的に物思いに耽る面も認められた。つまり、一見上手く疾病に適応しているように見え安心してしまいがちになるが、実際には、患者は透析治療を自分の中で同化できない状態であり、周囲の誰にもそのことが言えず内に篭もってしまう傾向もあることが判明した。Marlone-Crowne Social Desirabilityの解析では、高齢透析患者は社会生活に適応するために、自分自身を歪めている結果がでており、社会生活に適応するために患者は常にむりを強いられる傾向があり医療従事者は、このことを十分に理解する必要があることが明らかになった。
