本研究では、γDNAに関連した核内蛋白で、細胞の蛋白合成や増殖と密接な関連性を有するAg-NORs(Nucleolar Organizer Regions)の発現状況を、癌および周辺上皮で比較検討した。対象は、周辺上皮に組織学的な異常を認めた口腔粘膜扁平上皮癌31症例である。性別では男性19例、女性12例、年齢は42〜89歳、平均64.7歳であり、原発部位は舌16例、歯肉8例、口底5例、頬粘膜、口唇が各1例であった。癌の臨床進展度ではT0が1例、T1が8例、T2が18例、T3が3例、T4が1例、N0が23例、N1が7例、N2bが1例であった。周辺異常上皮の臨床型では、白斑型21例、白斑紅斑混在型5例、疣贅型2例、特に異常を認めなかったもの3例であった。病理組織学的所見では癌の大部分は高分化型扁平上皮癌であり、周辺上皮には、単純な過角化から中〜高度の上皮異形成を示すものまでみられた。Ag-NORsの検索方法は、生検または手術材料のパラフィンブロックより厚さ6μmの切片を作成、Howell and Blackのone-step法に従い銀染色後、1000倍油浸レンズ下で写真撮影し、細胞100個について核内銀染色顆粒(Ag-NORs)の分布状態と平均NORs数を算定した。その結果、正常上皮では、中〜大型円形のAg-NORsを1〜3個(2.1【.+-。】0.2)有するものが多く、癌(3.5【.+-。】1.0)では大きさ、形状が多彩であり、Ag-NORs数は増加していた。癌周辺上皮では一般に大型円形だが、ときに小型、不整型のものもみられ、Ag-NORs数は癌と正常上皮の中間的値(2.6【.+-。】0.7)を示すものが多く、なかには癌に類似したものも認められた。また、Ag-NORs所見と上皮異形度との関係、臨床型との関係では相関性は認められなかったが、位置的分類では、I型とII型に平均Ag-NORs数が高いものがみられた。

In this study, we compared the development of Ag-NORs(Nuclear Organizer Regions), cancer and peripheral epithelium in relation to γ-DNA, protein synthesis, proliferation and adhesion. 31 cases of squamous cell carcinoma of oral mucosa were identified. Sex: male 19 cases, female 12 cases, age: 42 ~ 89 years old, average 64.7 years old, origin: tongue 16 cases, teeth 8 cases, bottom of mouth 5 cases, buccal mucosa 1 case, lip 1 case. The clinical progression of cancer was T0 in 1 case, T1 in 8 cases, T2 in 18 cases, T3 in 3 cases, T4 in 1 case, N0 in 23 cases, N1 in 7 cases and N2b in 1 case. The clinical types of peripheral abnormal epithelium were white spot type in 21 cases, white spot red spot type in 5 cases, wart type in 2 cases, and special abnormal epithelium type in 3 cases. Pathological findings showed that most of the tumors were well-differentiated squamous cell carcinomas, peripheral epithelials, pure hyperkeratosis, moderate to high grade epithelialization. Ag-NORs detection method includes: preparation of 6μm thick sections of surgical materials, Howell and Black one-step method, silver staining, photography under 1000-fold oil immersion, calculation of distribution state and average number of Ag-NORs in 100 cells.
