口腔粘膜扁平上皮癌新鮮例のうち、癌周辺上皮に何らかの組織学的な異常が確認された症例を対象とし以下の検索を行なった。(1)臨床的検索性、年齢、部位、癌の臨床進展度、周辺粘膜の肉眼所見、癌との位置関係について検討した。その結果、口腔粘膜癌全体と比較して、性、年齢構成に有意差は認められなかった。部位では舌が約50%と多く、臨床進展度ではT2までの小さい症例が約80%を占めていた。肉眼所見では白斑が約65%と最も多く、位置関係では異常上皮内に癌が認められたもの(I型)が約50%、両者が互いに接するもの(II型)が約30%、非連続した部位に認められるもの(III型)が約5%、II+IIIが約15%であった。(2)組織学的検索実験材料にH-E染色を行い顕鏡した。その結果、癌の分化異型度は高分化で低異型のものが約80%を占めていた。周辺上皮の組織所見では異形成を欠くものが約10%、軽度異形成が約50%、中等度が約20%、高度が約20%であった。(3)p53蛋白の免疫組織化学的検索口腔扁平上皮癌43例を対象とし、抗p53抗体を用いた免疫染色を行なった。周辺上皮に異形成を認めた症例は17例、認めなかったものは26例であった。p53は27症例の癌部で陽性を示した。周辺上皮では、基底細胞側の1〜2層のみ陽性を示すものから、5層以上にわたり陽性所見を示すものまで種々認められた。また、3層以上の陽性を示したものは、異形成を認めたもので17例中15例、異形成を認めなかったもので26例中3例であり、上皮異形成がみられた症例に陽性率が高い傾向であった。
