咀嚼は健康な生活を維持するために欠かせない問題である。これまで、咀嚼に関して歯科的には虫歯(齲歯)や歯周病による咀嚼機能の低下に対する治療が重点的に行われてきた。しかし、最近では咀嚼時あるいは下顎運動時に伴う顎関節部疼痛、顎関節雑音及び閉口障害を主症状とする顎関節機能障害が増加し社会問題になっている。我々は1994年に歯科医療機関のない中国吉林省北方の農村部で歯及び顎関節の検診を行い、雑穀を主食とする住民に歯の咬耗が多く、齲歯や顎関節症状は極めて少ないことを確認した。さらに、この地方は産業、経済、交通状況などから考察すると、高度成長期以前の日本の社会環境を想定することができた。そこで、これまでの日本人の生活環境の変化が顎関節機能障害の発症に及ぼした影響を知ることを目的として、今回は産業、経済が発達し人口密度の高い中国の都市部で1995年に行った歯及び顎関節の検診結果をもとに農村部の検診結果と比較検討した。対象は年齢を25歳から40歳までの農村部185名(男性80名、女性105名)、都市部214名(男性102名、女性112名)であった。その結果、一人平均齲歯数、一人平均処置歯数および一人平均喪失歯数は農村部より都市部が2から3倍程度多くみられ、顎関節機能障害(雑音の発現率)は農村部は3%、都市部は25%で約8倍であった。これらのことから、齲歯が多く歯科治療を受ける機会が多い環境では、顎関節機能障害を生じる可能性が高く、顎関節機能障害発症に社会環境因子の関連性が示唆された。このことは、日本での顎関節機能障害の増加要因の一つとも考えられた。今後さらに調査を継続し顎関節機能障害の発症因子を解明したいと考えている。
