歯科臨床において、顎関節症患者は、痛みを訴えて来院することが多い。しかし、我々はその痛みを把握しにくいのが現状であり、顎関節症患者の痛みを客観的に把握することは機能障害の評価とともに必要不可欠である。我々はこれまで痛みを客観的に評価する方法としてVisual Analogue Scale(以下VASと略す)を応用し、歯痛の定量的測定や舌痛症の痛みの測定および治療効果の評価に有効であることを報告してきた。そこで、今回は、顎の痛みを主訴として東北大学歯学部付属病院を受診し顎関節症と診断された30名の患者に対して、痛みの質はマクギル疼痛質問表を用いて測定し、痛みの量はVASを用い、さらに治療後の痛みの経過をVASで測定した。その結果、(1)痛みの発現はあくびをする時(83.3%)、咀嚼時(75.0%)、開口時(75.0%)に多く認められた。(2)痛みの質については、瞬間的な、重苦しい、しこりがあるような、つかれきる、つらい、気になる、うっとうしい、動くと痛い、など、グレードの低い言葉が多く、痛みそのものとしては軽度であるが回数が重なることによって消耗感や苦痛といった情動面に大きな影響を与えていることが認められた。(3)痛みの量については、初診時VAS値の平均:54.0(S.D.±29.2)が、2週間経過観察後の再来時VAS値の平均:31.6(S.D.±23.7)と有意に減少した。この結果は初診時の検査、疾患の説明および生活指導が情動面に影響を及ぼすことを示し、経過観察の重要性が確認された。(4)治療による痛みの経過をVASを用いて測定することは治療効果の客観的把握に有効であった。今後さらにVASを用いた痛みの客観的評価法を臨床に応用したいと考えている。
