乳房温存療法をうけ退院後3カ月を経過した患者2名(女性。平均年齢55歳。TNM分類はT1aN0M0。病期分類は第I期)を分析対象とした。乳癌への罹患・治療体験では、術前は乳癌宣告、術式選択、再発の可能性、術後の上肢運動障害、術後の乳房や腋窩の変形、術創が、術後は退院後の放射線照射、タモキシフェン内服、再発の可能性、退院後の生活の注意が、退院後1カ月は再発の可能性、毎日の通院が、退院後3カ月は再発の可能性が苦悩を引き起こしていた。身体・生活の変化では、術前は食欲不振と不眠が、術後は不眠、上肢の運動障害、上肢運動障害に伴う不快感覚、腋窩と上腕内側の感覚鈍麻が、退院後は術後の苦悩に加え、上肢運動障害による生活の不自由、行動拡大に伴うむくみの出現、疲れ、照射部位の痛みと様相の変化が、退院後3カ月には上肢運動障害に関する問題は解決し、照射部位のかゆみ、疲れが苦悩を引き起こした。ボディイメージの変化では、術後の身体変化が苦悩を引き起こし、症状の緩和に伴い軽減していた。黒い切開創、消毒液による茶色、腋窩の創も苦悩を引き起こし、様相の改善に伴い軽減していた。変形した乳房は格好がいい、女としてはないよりはあったほうがいいと感じていた。家族関係の変化では、癌宣告により緊張感が高まり苦悩を引き起こしたが、夫との術式選択の検討、子供の励ましと家事協力により連帯感が高まった。退院後は家族が患者をいたわり協力して家事を行った。これは患者の癒しになったが,反面、苦悩を引き起こした。退院後3カ月には今までの家族関係に戻ってていた。社会活動・対人関係の変化では、術前は近所で噂される予感、社会への癌罹患の告知、治療のための仕事整理が、術後は近所で噂される予感、見舞客の同情的な言葉が、退院後は近所の噂、仕事や活動の再開が苦悩を引き起こしたが、退院後3カ月には近所の噂も軽減し仕事や活動が軌道にのり、苦悩は軽減した。

Two patients (female) were discharged from the hospital 3 months after breast tenderness therapy. The average age is 55 years. TNM classification T1aN0M0. The disease is classified as stage I. Breast cancer diagnosis and treatment, preoperative breast cancer diagnosis, surgical method selection, recurrence possibility, postoperative upper limb movement impairment, postoperative breast and armpit shape, surgical trauma, postoperative radiation exposure after hospital discharge, internal administration, recurrence possibility, attention to life after hospital discharge, recurrence possibility 1 month after hospital discharge, daily hospital discharge, recurrence possibility 3 months after hospital discharge, etc. Changes in body and life, loss of appetite and sleeplessness before surgery, sleeplessness after surgery, motor impairment of upper limbs, motor impairment of upper limbs accompanied by discomfort, numbness of armpits and inner wrists, increased pain after surgery after hospital discharge, motor impairment of upper limbs, loss of freedom of life, increased mobility accompanied by fatigue, changes in pain of irradiated parts, 3 months after discharge, the problem of upper limb movement impairment was solved, and the pain and fatigue of the irradiated part were caused. The symptoms are relieved and the pain is reduced.
