1.乳房切除術後うけた乳癌患者2名(女性。平均年齢47.0歳。既婚。子供あリ1名、なし1名。Stage I 1名、II1名。タモキシフェン内服)を対象に分析した。乳癌への罹患・治療体験の側面:乳癌の宣告、再発の可能性、ホルモン剤の内服等は苦悩を、病気をした体験は自己成長の感覚を引き起こしていた。身体機能,生活の側面:患側上肢の運動障害、患側上肢・乳房切除部・腋窩における不快感、日常生活の制約、身体不調等は苦悩を引き起こしていた。外見の側面:乳房の喪失、術創の様相は苦悩を引き起こしていた。夫婦関係の側面:夫の家事や育児への協力、夫の精神的支持は安寧を引き起こしていた。親子関係の側面:子供の動揺、子供に癌が遺伝する可能性は苦悩を、子供の家事への協力、子供の精神的支持は安寧を引き起こしていた。社会活動・対人活動の側面:他者の過剰な反応、役割遂行上での制約は苦悩を引き起こしていた。同病者の存在・つき合い、健常者の気配り・励ましは安寧と苦悩の2つの感情を引き起こしていた。親身な医療スタッフの存在は安寧を引き起こしていた。2.乳房温存療法をうけた患者5名の分析(平成10年度実績報告書)及び乳房切除術をうけた患者2名の分析より以下の知見を行た。1)乳房温存療法をうけた患者は多くの心理的苦悩を抱えており、その苦悩は時間と共に変化する。2)乳房温存療法をうけた患者の心理的苦悩の殆どは、乳房切除術をうけた患者によっても経験されるものであると考えられた。3)乳房温存療法をうけた患者が経験する心理的苦悩のうち、術式の自己決定や放射線治療により引き起こされる苦悩は、乳房温存療法をうけた患者にのみ認められた。また乳房の膨らみの残存や乳房の形の改善により引き起こされる安寧は、乳房温存療法をうけた患者にのみ認められた。4)本研究は分析対象が少ないため、対象数を増やして検討する必要があると考えられた。
