慢性疾患をもつ中学・高校生の飲酒・喫煙の実態と、その関連要因を明らかにするために、1)中学生・高校生の健康行動に関する測定用具の開発、2)慢性疾患をもつ中学・高校生の飲酒・喫煙の実態、3)2)と親の不安との関連について、を検討した。1)工藤(同大学母性看護)佐藤(保健学)と共に3県の中学生447名を対象に調査を行い、中学の健康行動を測定する尺度を開発した。この尺度の信頼性・妥当性の検討に関してまとめたものは投稿中である。高校生に対しても今後調査を行い、検討する予定である。2)国立療養所千葉東病院の小児腎外来にて調査を行った。調査質問紙はこれまでの研究成果を元に、療養行動、飲酒・喫煙の実態、病気に関連するストレス項目を含んだものを作成し、質問紙調査の後に面接を行い、今後も調査を継続する予定である。これまでのところ、対象は16〜20歳の30名であり、男・女とともに、高学年になるにつれて、友人と飲酒する機会が多く、それらを重要と考えていることがわかった。飲酒は疾病の重症度とは関係がみられず、健康な高校生と同程度の飲酒率で、動機や理由もほぼ同じと考えられた。喫煙が習慣化している者は男子のみであり、飲酒よりも経験者は少なかった。飲酒・喫煙の疾患に対する影響についての知識が不足しており、今後啓蒙活動や、教育・援助が必要と思われた。調査中に、高校中退し暴飲の見られるケースや、家族関係に問題があり、飲酒・喫煙が習慣化しているケース、実験的に大量の飲酒をするケースを見出し、個別的な援助の必要性を認めた。これらのケースは今後も、外来スタッフと共に関わりを持ち、援助の方向性を検討する予定である。3)親の不安に関しては、今回の調査では、親子を対として調査を行うことはできなかったが、子どもの飲酒・喫煙の実態を知りつつも黙認する・叱責するのみで、それに対する効果的な関わりはほとんどなく、困惑する様子が伺えた。上記の問題ケースを中心に、今後も事例検討を重ね、家族への援助や、飲酒・喫煙行動を予防するような家族の関わりを明らかにしたいと考えている。
