项目摘要

【目的】甲状腺癌は乳頭癌・濾胞癌に大別され、前者は正常甲状腺組織よりde novoに発生し、後者は濾胞腺腫を経て癌化するとされている。これらの進展過程に関与する特定の癌遺伝子・癌抑制遺伝子との密接な関係は特に濾胞性腫瘍未だ捉えられておらず、他臓器とは異なる発癌・進展過程が推測されている。甲状腺濾胞性腫瘍の標本を詳細に観察すると、比較的異型性の少ない腫瘍組織の中に、細胞密度・核/細胞質比の高い細胞集団が存在することが一部の例で見出され、このような部分の細胞が被膜浸潤・脈管侵襲などに関与する可能性が考えられた。本病変が甲状腺において早期癌に相当する一つの段階ではないかとの想定のもと、その生物学的性格の特徴を捉え濾胞癌における多段階発癌機構を吟味したい。【方法】伊藤病院における甲状腺手術材料連続2210症例中、病理組織学的に濾胞癌と診断した42例のうち、結節内結節が存在し、同部で浸潤像を認める症例5例を対象とした。こうした領域を構成する細胞集団が周囲と性格を異にするか否かを検討するため、昨年度のPCNA labelling indexの測定に引き続き、今回はDNA contentに着目し以下の操作を行った。即ち、対象例の切片を50μm厚に薄切し、3領域(非腫瘍部・腫瘍中心部・浸潤部)に分離。それぞれを脱バラフィンし、propidiumlodideで染色後、Eplcs Profile II flow cytometer(Coulter)にて各領域のDNA contentの計測を行った。【結果】甲状腺濾胞性腫瘍の中には、腫瘍結節中に構造上あるいは細胞形態学上異なった性格を有する結節内結節が認められ、その小結節の部分において浸潤像を有している例が少数ながら存在した。こうした領域はPCNAでの結果と平行して、DNA contentも増加している傾向が認められた。【考案】甲状腺濾胞性腫瘍のprogressionには増殖能の面でheterogenousな性質を有するsubcloneの出現が関与している可能性が示唆された。

[Objective] Thyroid cancer is a combination of papillary carcinoma and follicular carcinoma, the former is a new development of normal thyroid tissue, and the latter is a cancer development. The progression of cancer is related to specific cancer vectors, cancer suppressor vectors, and specific cellular tumors. A detailed examination of thyroid cysts showed that there were relatively few heteromorphisms in the tumor tissue, cell density, high nuclear/cytoplasmic ratio, and the presence of cell clusters in some cases. The disease is characterized by early thyroid cancer, multiple stages of cancer development, and biological characteristics. [Methods] Among 2210 cases of thyroid surgery in Ito Hospital, 42 cases were diagnosed as filter cell carcinoma by histopathology, and 5 cases were diagnosed as infiltration in the same part. The following operations are performed on the cell population composition, cell cycle and personality differences, PCNA labeling index determination, and DNA content determination. In other words, the slices of the two samples were cut thinly at 50μm thickness and separated into three areas (non-tumor area, tumor center area and infiltration area). DNA content measurement in various fields by Eplcs Profile II flow cytometer(Coulter) [Results] In thyroid cysts, there were some structural differences and some morphological differences in nodules and nodules. The DNA content of PCNAs increases with the increase of DNA content. The relationship between the heterogeneous nature of proliferation and the possibility of subclone occurrence in the progression of thyroid cysts is discussed.
