内因性精神病の病因に遺伝要因が関与していることは、臨床遺伝学的研究から明らかにされている。しかし精神分裂病、感情障害のいずれにおいてもその遺伝様式は明らかではなく、また分子遺伝学的手法を用いた連鎖、相関研究も行われているが、いまだ一致した結論に至っていない。その一因として遺伝的異種性の存在が考えられる。そこで今回我々は、満田らをはじめとする臨床遺伝学的な研究から、遺伝的背景が異なるとされている非定型精神病を対象として、抗精神病薬の薬理学的特徴として注目されているドーパミンD2受容体(DRD2)に注目し、非定型精神病とDRD2遺伝子との相関研究を行った。さらに豊田らの分類による非定型精神病の下位分類のなかのType1についても相関を検討した。DRD2遺伝子のなかで、5'flanking regionのA-241G多型、-141C Ins/Del多型、エクソン7、コドン311のアミノ酸ではセリンからシステインへのミスセンス変異、Taql A多型を用いて非定型精神病、下位分類のType1と正常対照群との間の相関を検討した。TaqlA多型、セリン、システィン多型、-141C Ins/Del多型では遺伝子型および遺伝子頻度において、非定型精神病と正常対照群、Type1と正常対照群の間に有意差は認めなかった。A-241G多型では遺伝子型および遺伝子頻度において、非定型精神病と正常対照群の間には有意差は認めなかったが、Type1と正常対照群の間で有意差を認めた。(p<0.05)今回の調査では、非定型精神病のType1と5'flanking regionのA-241G多型との間で弱い相関を認めた。有波らによるとA-241G多型のごく近傍にある-141C Ins/Del多型のうち-141C Delアレルを組み込んだluciferase reporter plasmidはwild haplotypeである-141C Insアレルに比べると発現量が落ちるが、A-241とG-241との間にはluciferase reporterplasmidでの発現量に差はないとしており、今回の我々の結果が直接非定型精神病の病因の説明することにはならない。5'flanking regionの近傍をさらに探索していく必要がある。今後さらに症例を集積し、また他の候補遺伝子に対する検索も必要である。
