膀胱癌におけるE-cadherin、cathepsin Dおよびnm23蛋白の臨床的意義を免疫組織化学とEG遺伝子mutationの有無により検討した。膀胱全摘除術を試行した94症例のパラフィン切片を用いてE-cadherin(done6F9)の免疫染色性、cathepsin Dの免疫染色性、nm23蛋白の免疫染色性、Ki67(doneMIB1)の免疫染色陽性率、TUNEL法によるアポトーシス指数(Al)、Feulgen染色による5cexoeedngrate(5cER)を評価した。E-cadherinは細胞膜染色性(EC-M)と細胞質染色性(EC-C)を別々に評価し、EC遺伝子のmutationの有無はexon6から14までの各exonを含む領域をPCR法で増幅しSSCP法にて解析した。94例中59例(62.3%)でEC-Mが減少し、19例(20.2%)がEC-C陽性であった。EC-Mの減少は組織学的深達度および異型度、Ki67陽性率、Alおよび5cERとの間に正の相関を認めた。EC-C陽性率は5cERとのみ正の相関を認めたが、EC-C陽性19例ではEC遺伝子のmutationは観察されなかった。Cathepsin D陽性率は組織学的異型度、深達度および5cERに対して負の相関を示した。一方、nm23蛋白陽性率は組織学的異型度と正の相関を認めたが、深達度と5cERとの間には有意な関係を認めなかった。多変量解析では予後に関してEC-MとEC-Cはともに有意な規定因子ではなかったが、生存率では組織学的深達度とcathepsin D陽性率が、非再発率ではKi67陽性率が独立した規定因子であった。以上の結果から膀胱癌におけるE-cadherinの臨床的意義は組織学的所見、cathepsin D陽性率および増殖能に比較し乏しいと考えられた。また膀胱癌におけるE-cacherin遺伝子の異常の頻度は低いと考えられた。

Clinical significance of E-cadherin, cathepsin D and nm23 protein in bladder cancer The immunostaining of E-cadherin(done 6F9), cathepsin D, nm23 protein, positive rate of Ki67(done MIB1), TUNEL staining index (Al) and Feulgen staining were evaluated in 94 cases of total cystectomy. E-cadherin was analyzed by PCR and SSCP in different exons of cell membrane (EC-M) and cytoplasm (EC-C). EC-M decreased in 59 cases (62.3%) and EC-C was positive in 19 cases (20.2%). EC-M reduction is associated with histological depth, heterogeneity, Ki67 positivity, Al and 5cER. EC-C positive rate was 5cER and EC-C mutation was detected in 19 cases. Cathepsin D positive rate was negatively correlated with histological heterogeneity, depth and 5cER. The positive rate of nm23 protein was higher than the positive rate of nm23 protein. Multi-quantitative analysis is related to EC-M and EC-C. It is intended to specify factors, survival rate, histological depth, cathepsin D positive rate, non-recurrence rate, Ki67 positive rate and independent specification factors. The clinical significance of E-cadherin in bladder cancer was compared with histological findings, cathepsin D positive rate and proliferation ability. The abnormal frequency of E-cacherin gene in bladder cancer is low.
