膀胱腫瘍におけるサイトケラチン19発現に関する基礎的検討

浸潤性膀胱腫瘍患者(T3以上)において、化学療法前後における腫瘍の大きさ、異型度、浸潤度、モノクロナール抗サイトケラチン19抗体(PC11417 コスモバイオ社)による免疫組織化学的染色の程度について比較検討を行った。まず、経尿道的膀胱腫瘍切除術により採取された検体のHE染色から、腫瘍の異型度、浸潤度を判定し、浸潤性膀胱腫瘍(T3以上)と判定された患者20例に対して。モノクロナール抗サイトケラチン19抗体による免疫組織化学的染色を行い、その染色程度と染色細胞陽性率を判定した。これらの患者に化学療法(CMV療法:CDDP100mg/mm^2、MTX30mg/mm^2×2回、VBL4mg/mm^2×2回を1ク-ルとして2ク-ル施行)を行い、その後再手術により採取された検体を用いて同様にHE染色と免疫組織化学的染色を行い、検討した。腫瘍が化学療法により縮小した症例では、HE染色にて異型度、浸潤度の改善をみた症例もあり、化学療法が無効であった症例では、HE染色にて異型度、浸潤度の憎悪をみた症例もあったが、モノクロナール抗サイトケラチン19抗体による免疫組織化学的染色では、化学療法前後で特に染色程度と染色細胞陽性率に有意の差は認められなかった。また、同時に経尿道的膀胱腫瘍切除術前より化学療法を経て再手術までの尿中可溶性サイトケラチン19フラグメント(以下シフラ21-1)を経時的に測定した。浸潤性膀胱腫瘍患者において、経尿道的膀胱腫瘍切除術後に尿中シフラ21-1は低下し、術後7日目で術前に比べ有意の低下をみた(p<0.01)。その後化学療法により腫瘍の縮小した症例では尿中シフラ21-1の再上昇は認められなかった。化学療法が無効にて、腫瘍が増大した症例では尿中シフラ21-1の再上昇を認めた。
