粘膜骨膜弁法にて行った手術は、上顎骨の劣成長、歯列弓の瘢痕拘縮および口蓋容積の減少を引き起こすことが明らかになっており、現在、歯列矯正、bone graftなど上顎歯列弓に対する治療が主となっている。しかし、上顎歯列の狭小化、浅い口蓋、鼻中隔の変形により鼻呼吸の障害などを引き起こし、舌の位置的変化による下顎歯列弓への影響が多かれ少なかれ存在することが考えられる。これは、第一には上顎歯列弓の影響が咬合を介して下顎歯列弓に影響を及ぼすという考え方と、第二には上顎歯列弓形態の異常に起因する舌、粘膜、口腔周囲の筋肉の不調和な活動といった機能の異常が間接的に下顎歯列弓形態に少なからず影響を与えている結果と推察される。第一の点に関しては、当科における報告において、上顎歯列弓形態の異常が咬合を介して下顎歯列弓形態に影響を及ぼしていることが示唆されている。今回、上顎舌房容積の減少によって引き起こされる舌を介した下顎歯列弓への影響を明らかにする目的で本研究を行い以下の結果を得た。1. 口蓋裂手術群の上顎舌房容積は健常児に比較して有意に小さかった。2. 上顎舌房容積小群と大群の下顎歯列弓長径、幅径は健常児に比較して有意に小さく、歯冠軸の傾斜度においても健常児に比較して有意に小さく歯冠軸は大きく舌側に傾斜していた。3. 上顎舌房容積と下顎の歯冠軸の頬舌的傾斜度との相関は認められなかった。以上の結果より、術後の下顎歯列弓の形態変化は、上顎舌房容積の減少による低位舌によるものよりは、むしろ咬合状態の変化が咀嚼筋動態を変化させ下顎骨に影響を与えるものと推察された。

Mucoperiosteal surgery is the main treatment for maxillary growth, scar contraction of the dental arch, and reduction of oral cavity volume. Narrowing of the maxillary dentition, shallow opening of the mouth, shape of the nasal septum, obstruction of nasal respiration, increase of the incidence of nasal respiration, change of the position of the tongue, and the influence of the mandibular dentition arch are more or less present. The effects of the first and second maxillary dental arches on occlusion, mandibular arch, and the causes of abnormal tongue, mucous membrane, and oral muscle movements were investigated. The first point is related to the changes in the morphology of the maxillary arch and the changes in the morphology of the mandibular arch. The results of this study are as follows: 1. The effect of mandibular arch on the reduction of maxillary and mandibular volume 1. The volume of upper lingual chamber in cleft palate surgery group is relatively small. 2. Maxilloglossal chamber volume small group and large group of mandibular dental arch length diameter, amplitude diameter and healthy comparison, intentionally small, coronal axis tilt and healthy comparison, intentionally small, coronal axis tilt and large lingual tilt 3. The volume of the upper jaw and the inclination of the mandibular crown axis are related. The results showed that the changes of mandibular arch morphology, the decrease of maxillary lingual chamber volume, the changes of occlusal state, the changes of masticatory tendon dynamics, and the changes of mandibular arch morphology were observed.
