项目摘要

遠春期女性の約80%が月経前症候群(Premenstrual syndrome:PMS)を示して周期的に半健康状態に陥ること、体内の鉄代謝関連物質の栄養状態を評価すると、貯蔵鉄が潜在的に欠乏した状態において頻度高く発現することを報告してきた。この場合、医療の対象外であることから健康・栄養教育が試みられるべきだが、その効果評価には主観的のみならず客観的指標が必要となる。そこで、月経周期性を伴う不定愁訴の出現と、できるだけ無侵襲に測定・した栄養状態の変動との関連を検討し、健康・栄養教育の効果評価を行うのに妥当な客観的指標であるか検討を行った。(1)出現頻度の高いPMSである「黄体期後半の甘味嗜好・摂取量の増大」の背景として基礎代謝量の変動を仮定し、健康(n=5)あるいは潜在性鉄欠乏状態(n=4)の思春期女性を対象に、2月経周期に渡り1または2日おきに細谷式携帯用簡易熱量計を用いて安静時のエネルギー代謝量の測定をしたところ、鉄の栄養状態に関わらず黄体期後半にやや高くなる傾向が見られたが、有意な差異ではなかった。(2)不定愁訴と関連するといわれるV.B_6濃度を、思春期女性(n=34)の黄体期ならびに卵胞期における24時間蓄尿中の4-ビリドキシン酸で検討したが、無侵襲条件は満たすものの24時間蓄尿の精度保持が困難であった。現在、侵襲を最低限にするシステムとして手指血血漿ピリドキサル5'リン酸濃度を測定し、静脈血中との差異を検討中である。(3)健康(n=41)あるいは潜在性鉄欠乏者(n=26)の思春期女性を対象に、PMSの発現を関連する栄養状態の変動は自らの生活習慣に基づくことを認識・学習することから始まる健康・栄養教育「ゴール設定プログラム」を実施したところ、セルフ・エフィカシイ(自己有効性)の高い人程生活習慣と不定愁訴の改善が顕著で、教育6ヶ月後まで効果が持続しプログラムは有効であった。
