健康成人の尿においてトリメチルアミンの検出をガスクロマトグラフ(GC)器機を用いて試みたが、標準ガスの発生可能な最小濃度以下のものがほとんどであった。そこでGC-質量分析計(MS)での検出に切り替えた。しかし、測定濃度の再現性も悪く、尿中からの検出は困難であった。今後、健康成人で検出する場合は、現在使用しているGC-MSのカラムをプロットカラムなどに変更することや、検体保存方法の改善、クレアチニン補正の適正化に留意したいと考えている。透析患者についても、尿中のトリメチルアミン量の測定が困難であったために、呼気の官能試験を内科医1名、歯科医2名で行った。官能試験の結果は0(臭いなし)から5(強い悪臭)までの6段階のスコアで表した。3名の判定者の2名以上の一致をもって、その患者のスコアとした。透析導入予定の患者5名において導入前はスコアがそれぞれ1、1、2、2、3であったが0、1、1、1、2と減少した。また、その臭いの種類は、官能試験では、「明らかに口臭ガスとして広く認知されている揮発性硫化物(メチルメルカプタン、硫化水素)と違う種類の臭いがする」と、3名の判定者の意見が一致した。さらに、腎疾患患者を多数診察している医師3名にインタビュー調査をしたところ、「透析導入前のBUNの悪化した状態では独特の口臭があり、透析を導入すると改善していく傾向があり、これは患者も家族も内科医も頻繁に経験する」とのことであった。これまで、歯科臨床経験上、「透析患者は独特の口臭がする」と考えていたが、今回の調査で「腎臓障害により、口臭は悪化し、透析によりそれが改善する」可能性が示唆された。ただし、この腎障害患者の口臭原因ガスが、続発性トリメチルアミン尿症のトリメチルアミンであると断定するには、さらなる検討が必要である。

This is the result of clinical studies on dentistry,"dialysis patients have unique halitosis," and the possibility of "renal impairment, halitosis, and dialysis improvement" in this study.
