わが国では、歯科定期健診(早期発見)による口腔疾患(とくに歯周疾患)の予防システムの確立が十分ではないのが現状である。しかも集団歯科検診等を含めた歯科定期健診のしくみが、地域においてうまく機能するかどうかは、サービスを受ける側、すなわち地域住民の自主的な行動に依存する面が大きい。今回の申請の目的は、地域住民のニーズに合った、より現実性のある歯科医療サービスを提供するシステムを確立するための一助として、歯科健康診断を受診するという行動(受診行動)について多角的な検討を行なうことである。本年度は、広島旧市内に在住の年齢20歳以上の人から無作為に抽出した1,200人を対象に、歯科健診を含めた歯科の受診状況及び歯科受療に対する意識についての郵送調査を行なった.(回収率53.4%)。対象者は、旧市内各区より4投票区を無作為抽出し、さらに登録者数を勘案して1区合計300人を乱数により無作為抽出をして、合計1200人となるようにした。調査票は、KaslとCobbによる保健行動モデルにもとづき、保健行動に関する質問、現在の歯科受診状況についての質問を作成した(計46問)。その結果、全体の約7割が『かかりつけの歯科医がある』と答えた。一方、『定期的に(1年に1回以上)歯科健診を受けたり、歯石を取ったりしている。』と答えたのは全体の約24%であった。さらに、 『あなたが、歯科医院を受診する主な動機は何ですか。』という質問に対して、約8割の人が“口の中に痛みやトラブルが生じた場合のみ"と答え、“口の中がサッパリして気持ちいいので(積極的に)行く"と答えた者は、わずか2%にすぎなかった。現在、歯科受診における保健行動モデルを提案するために、これらの因子を目的変数、歯科受療に対する意識や受診行動を説明変数として、これらの因子を分析中である。

The status quo of prevention of oral diseases (peri-dental diseases) is very important in the establishment of regular dental visits (early detection) in China.
