広島市はおよそ50年前の原爆投下で歴史上、類を見ない人的、物質的被害を受けた。本研究では広島市内某健康センターで被爆者定期健康診断を受診した50〜87歳の200名(男性42.6%、女性57.4%)を対象に歯科健診と歯科保健行動に関する質問紙調査(被爆50年後の歯科実態調査)を行なった。歯科健診の受診率は20%で、50〜54歳の女性の受診率はやや低かった(12%)。平均年齢は男性65.1歳、女性66.5歳で、被爆者全体の平均年齢とほぼ一致した。1.歯科保健行動:ブラッシング指導を希望すると答えた者は全体の56%を占めた。一方、約2/3の者が、老人になると入れ歯になるとは仕方ないと答え、歯ブラシだけで歯槽膿漏の予防はできないと思った。他の質問結果からも歯の健康保持には意欲的である反面、口腔清掃に関する知識の曖昧さがうかがわれた。2.口腔内自覚症状:冷水痛、歯の動揺、歯肉腫脹、噛みにくい等について全体の25〜30%が症状を自覚していた。歯と歯の間に食べ物が詰まるという症状を有する者は80%を占めた。男女間で差は認められなかった。3.口腔内状況:厚生省歯科疾患実態調査(H5年度)を元にした各年齢ごとの平均値と比べ、平均現在歯数は21.2本と多く、未処置歯数は1.2本と少なかった。無歯顎者は200名中6人であった。歯周疾患についてはほぼ全員が口腔清掃の改善と除石の処置を必要とした。なお被爆距離による口腔内状態の違いは男女ともにみられなかった。以上より、今回歯科健診を受診した被爆者においては健康管理意欲が高いが、一方では歯周処置を必要とする割合は高く、今後健康教育等により口腔に関する意識を高めていくことが大切と思われた。また今回の結果が被爆者の全体像を示すとは言い難い。今後の被爆者歯科保健医療を考える上で受診しなかった80%の者に対し,再度受診を促すなどの対応が必要と思われた。
