(1)局所壁運動指標の妥当性の検討:胸部心臓ファントムを用いてイメージング実験を行った。不動のファントムを正常人の心拍に同期させて撮像した。QGSソフトウエアを用いて算出したwall motion(以下WMと略す)は実験では最大で7%であった。収縮末期・拡張末期の左室心筋容積の変化率(以下VFと略す)は最大で5%であった。Wall motionは2次元のパラメータであるのに対して心筋容積は3次元のパラメータであり、わずかだが局所壁運動をより正確に反映していた。心筋容積あたりの心筋カウントの変化率(以下CFと略す)は実際には変化がないが実験では最大で25%となっていた。実際のヒト心筋では収縮するとCFは大きくなるがこれに見合う実験モデルについては現在検討中である。(2)正常例での検討:冠動脈造影にてAHA分類75%以上の有意狭窄を認めず、かつ左室造影にて壁運動異常がない患者3名(全例冠攣縮性狭心症患者)に安静時にTc-99m tetrofosminを静注し、心電図同期心筋血流シンチグラフィを施行した。WM、VF、CFいずれも心基部で値が小さく心尖部で値が大きい傾向にあった。心狭部における各指標のばらつきの程度はCFが最も小さくWM、VFは同程度であった。また、特に器質的疾患がない症例でも心室中隔の壁運動は自由壁に比し低下していることが判明した。WMは特に小さい心臓では心室中隔で値が低くなっていたが、VF、CFは心室中隔の数値は他部位とそれほど差はなかった。よってこの数値を用いることで心室中隔の壁運動も一律に評価できる可能性があり局所壁運動の評価が容易になると考えられた。
