近年,外分泌腺混合腫瘍の新しい腫瘍形態として、筋上皮由来の腫瘍細胞が優位なmyoepitheliomaという疾患単位が注目されている。さらに、従来のmalignant counterpartとしての悪性混合腫瘍・腺癌・腺様嚢胞癌に加えて、malignant myoepitheliomaの存在も報告されている。しかしながら、涙腺部上皮性悪性腫瘍は頻度が少ないため、十分に由来細胞の検討は成されていない。本研究はわれわれの教室で多数保管されている眼窩腫瘍の病理標本のうち、涙腺部上皮性腫瘍、特に悪性腫瘍の標本を用い、病理診断の再評価をするとともに、免疫組織化学的に筋上皮由来の腫瘍細胞の存在ならびにその割合を検索する。対象は過去17年間に当教室にて摘出された涙腺部上皮性悪性腫瘍5例のパラフィンブロック保存サンプルに対し検討を加える。当初、なされている病理診断は多形腺腫に合併した多形腺癌1例、癌肉腫1例、腺癌2例、腺様嚢胞癌1例である。そのうちの1例につき今回病理学的免疫組織化学的に検討した。ヘマトキシリンーエオジン染色では正常の涙腺構造がみられ、その腺葉の周囲より形態が崩れていくような部分を伴いその形態の崩れの部分より続く細胞が脱分化を示し、紡錘、類円形の肉腫様の未熟な所見をみとめた。免疫組織染色においては全上皮に陽性を示すEMAと平滑筋に特異性の高いα-SMAに腫瘍細胞が陽性であった。また電子顕微鏡所見でdesmosomeがみられたことなどより腫瘍が筋上皮細胞由来とおもわれた。腫瘍細胞の由来が腺上皮優位か筋上皮優位かをみるためductal formationを検索したが今回の場合単位観察野あたりの出現頻度は5%以下であり、このことからも筋上皮細胞由来のmalignant myoepitheliomaと考えられた。この研究結果は第33回眼科臨床病理組織研究会、及び日本眼科学会雑誌105巻1号に発表した。今後,更に他の症例の再評価をし、病理学的、免疫組織化学的に検討していく。

In recent years, the morphology of mixed tumor of exocrine gland and tumor cells derived from tendon epithelium have been studied. The presence of malignant myoepithelioma was reported in the present study. Epithelial lesions in the glandular region are more frequent than those in the cellular region. The present study covers the pathological examination of orbital cysts, glandular epithelial cysts, and atypical cysts, reassessment of pathological diagnosis, and immunohistochemical examination of the presence and association of tumor cells derived from muscle epithelium. In the past 17 years, 5 cases of glandular epithelial tumors have been collected and discussed. At the beginning, the pathological diagnosis was pleomorphic adenoma combined with pleomorphic adenocarcinoma in 1 case, carcinomatous adenoma in 1 case, adenocarcinoma in 2 cases and adenocarcinoma in 1 case. A case of this disease was examined by immunohistochemistry. The structure of the gland is normal. The morphology of the gland is abnormal. The morphology of the gland is abnormal. The dedifferentiation of the gland is abnormal. The spindle is abnormal. Immunohistochemical staining showed that the tumor cells were positive for smooth muscle. The origin of the epithelial cells of the tendon was observed by electron microscopy. The origin of tumor cells is from glandular epithelium to tendon epithelium. The frequency of occurrence of tumor cells is less than 5% in this case. The results of this study were presented at the 33rd Society of Clinical Pathology of Ophthalmology and the Japanese Academy of Ophthalmology, No. 105, No. 1. In the future, further re-evaluation of other cases, pathological and immunohistochemical studies will be carried out.
