はじめに,基礎的検討としてプラーク中のミュータンス連鎖球菌群の量的評価をSite Strip(以下SS)で行うにあたり,採取プラーク量(肉眼的判定量)とSSの判定値について,24時間の歯口清掃停止後,ヒト口腔より採取した同一のプラークを6ないし8つに分割し,検討した結果,肉眼所見で4倍以上の差があり,SM寒天培地を用いたCFUで1オーダーの差がみられるような症例においても,SSの判定値にはほとんど差が認められなかった。そこで,平成11年度には,成人125名(21歳から25歳)を対象にSS(口腔内の隣接面4部位より採取)およびStrip mutans(以下SM)の両法によりミュータンス連鎖球菌群の量的評価を行い,あわせて口腔診査を行った。この結果,以下の結論を得た。1,同一口腔内のSSの4部位の評価値のうちの最高値を用いることにより,SSはSMと同様に一口腔の指標として有用である。2,被検歯面の齲蝕経験とSSの評価値との間には関連性が認められた。さらに,平成12年度は,0歳から6歳までの小児189名を対象に成人の場合と同様に両法を応用し,あわせて口腔診査を行い,検討を試み,以下の結論を得た。1,成人の場合と同様に,一口腔の指標としてSSの最高値に有用性が認められ,また,通常の口腔診査では捉えにくい隣接面の状況を簡便に把握出来る可能性が示唆された。2,0から2歳までのSSの有所見者,SMの有所見者および齲蝕罹患者の月齢の増加による推移について累積度数を求め近似曲線により検討を行ったところ,SSの有所見者,SMの有所見者,齲蝕罹患者の順に推移する現象が認められた。以上より,低年齢児の齲蝕リスクを考慮する際には唾液よりもむしろブラークを用いたほうが有用性が高いことが考えられた。
