项目摘要

経管栄養中の重症心身障害者における適切な口腔ケアへの指針を明確にすることを目的として,平成12年度は経管栄養者へのプロフェッシヨナル・オーラル・ケアを3か月間実施し,口腔内環境および全身状態に与える影響について検討した。1.ベースライン調査調査開始前のベースラインとして,経管群7名について,以下の項目につき調査した。(1)清掃状況Plaque Indenx (PlI),Gingival Index (GI),Caliculus Index (CI)(2)口腔内環境唾液pH,S.mutans,Lactobacillus,C.albicans,歯周病原因菌(P.gingivalis,B.forsythus,T.denticola)(3)過去1か月間における38.0度以上の発熱回数2.プロフェッシヨナル・オーラル・ケア開始後の変化(1)清掃状況PlI:1か月目では,ベースライン時と比較して有意に低くなったがそれ以降では変化はみられなかった。GI:3か月間継続しても変化はみられず,軽度歯肉炎に罹患していた。CI:3か月間継続しても変化はみられなかった。(2)口腔内環境唾液pH:ベースライン時は中央値7.7であり3か月後の中央値は7.4で統計学的に有意差はみられなかった。口腔内細菌:ベースライン時と3か月後を比較して,いずれの口腔内細菌も減少傾向ではあったが,統計学的に有意差はみられなかった。(3)発熱回数ベースライン時は平均3.5±1.4回であり,1か月,2か月後と徐々に減少傾向を示し,3か月後には2.1±0.3回と有意に減少していた。
