小児ランゲルハンス細胞組織球症(LCH)は、その約85%の症例に難治性溶骨性病変を合併する未だ病因不明の組織球増殖性疾患で、破骨細胞活性化作用を持つサイトカインの存在が報告されている。LCH5例に破骨細胞抑制作用を持つbisphosphonate, COX2inhibitor併用投与したところ骨病変の著明な改善を認めた。その治療効果を検討し、in vitroでの破骨細胞形成能、機能を評価し、bisphosphonate、COX2inhibitorの抑制効果を評価した。方法は難治性骨病変を有するLCHの5症例にbisphosphonateを投与し、3D-CTおよび尿中NTx測定より骨病変の変化を評価した。また、LCH患者の末梢血単核球を用いて破骨細胞形成と骨吸収能に対するbisphosphonateの効果をin vitroで検討した。結果は(1)LCH患者の骨病変部位にTRAP陽性破骨細胞の存在を確認した。(2)難治性骨病変を有するLCH2症例にbisphosphonate COX2inhibitorを投与し3D-CTにて骨融解像が著明な改善を示した。(3)bisphosphonate COX2inhibitor投与により尿中NTxは骨吸収像の改善と比例して低下し、投与前の17.8%にまで低下した。(4)LCH患者由来末梢血単核球からの破骨細胞形成および骨吸収能はbisphosphonate、COX2inhibitorの投与により抑制された。【考案】LCH病変部位にはTRAP陽性多核細胞が存在している事、及び臨床的観察から、従来有効な治療法がなかったLCHの難治性骨病変に対し、破骨細胞制御という観点からのbisphosphonateCOX2inhibitor療法の有効性が示され、LCH骨病変の病態形成に破骨細胞が深く関与していることが判明した。

In the treatment of osteolytic disease (LCH) and osteolytic disease (Osteolytic disease) with unknown etiology, the cellular activation of osteoclasts and osteoclasts has been reported. In the case of LCH5, the inhibition of osteoclast cells maintained bisphosphonate, and COX2inhibitor was used to improve the recognition of bone disease. The results showed that the formation energy of bone-breaking cells, the ability of in vitro, bisphosphonate and COX2inhibitor could inhibit the formation of bone cells. Methods in the treatment of bone disease, there were 5 cases of LCH, bisphosphonate and 3D-CT. NTx in urine was determined. The peripheral blood nuclear globules of patients with chronic hepatitis B and LCH were formed with bone resorption cells to form bone aspiration energy, bisphosphonate cells, and in vitro cells. Results (1) there was a confirmation of TRAP osteoclast in the bone disease site of LCH patients. (2) in the treatment of osteopathy, there were cases of LCH2's disease, bisphosphonate COX2inhibitor and bone fusion image of 3D-CT. (3) the ratio of bisphosphonate COX2inhibitor to urine NTx bone aspiration was improved, the proportion was low, and the former was 17.8% and 17.8% respectively. (4) in patients with LCH, peripheral blood nuclear globules, osteoclasts, bone resorption, bisphosphonate, COX2inhibitor and inhibition of bone formation were observed. [case study] there are TRAP multinucleated cells in the site of LCH disease, as well as the observation of the bed, the treatment of bone disease, the control of osteoclast cells, the presence of bisphosphonateCOX2inhibitor, the formation of bone disease, the depth of osteoclast cells and the diagnosis of bone disease.
