本研究は、うつ病のHPA系機能異常におけるグルココルチコイド(GC)受容体異常を検討するにあたり、従来の研究報告がうつ病患者の末梢血リンパ球のリガンド結合特性を検討し、受容体数と親和性の減少を報告しているものの、未だ確定できていない状況にあることに着目し行われた。我々はGC受容体のより精密な分子学的検討としてαとβの2つのisoformの量比がリガンド結合特性に反映されている可能性を検討した。既に正常人の種々の臓器で2つのisoformのRNA発現量比が測定され報告されているが、末梢血リンパ球に関しては報告が無く、まずリンパ球でのRNA発現量比を測定する系を確立し、続いて対照群と患者群のリンパ球でのαとβのisoformのRNA発現量比を決定することとした。人血液を低浸透圧で溶血した後に遠心分離して得られたリンパ球を蛋白変性剤中でホモジナイズし、RNA分離用スピンカラムで遠心分離して総RNAを得た。総RNAを逆転写し、上流はαとβに共通のものを、下流はそれぞれに固有のものをPCRプライマーとして加え、競合的PCR反応を行い、それぞれ477bpと366bpのPCR産物を得た。競合的PCRのカイネティクスを知るため、αとβのcDNAを100:1、10:1、1:1、1:10、1:100の比で混合し競合的PCRを行い、αとβの量比と一定のRT-PCR産物量を得られるサイクル数の関係を検量線として作製した。対照群4人と患者群4人(単極性2人、双極性2人)の一定のRT-PCR産物量を得られるサイクル数を検量線に外挿し、αとβのRNA発現量比を求めた。対照群のリンパ球では他臓器での報告ととほぼ同じβ/α=平均0.13%を得た。一方、患者群ではβ/α=平均0.05%と低下していた。今後は、患者群のβ/αの低下を、治療経過や重傷度などの条件を変え検討していく方針である。
