项目摘要

平成6年度から8年度の3年間に、県立岡本台病院で措置鑑定(2次鑑定)を受けた精神障害者230名、および警察署など栃木県内の他所で措置鑑定を受け措置一時預かりで県立岡本台病院に入院した精神障害者58名の計288名を対象として、措置鑑定時の状態および平成11年度末の状態の調査を行い、精神分裂病患者の暴力発動に関して、以下の知見を得た。i.妄想や幻覚に支配された他者に対する激しい攻撃は、比較的高年齢でなされ、発症後長い年月を経た中年期に発現する傾向があった。ii.攻撃時の緊張病状態および攻撃の方向性が定まっていることは、攻撃行為後の速やかな病状回復と相関した。iii.攻撃行為後に速やかな病状回復が見られる事例では、病前の社会適応がよい傾向が認められた。iv.大多数の攻撃行為は病勢増悪時に見られ、通院の中断や怠薬が大きく影響していた。通院中断の末に病勢増悪して措置入院に至った事例の通院中断期間は、3ヵ月以上1年未満が大半を占めるため、PSW等の充足等により精神障害者の受診行動を把握し、精神医療からの脱落を防ぐことができれば、精神障害者による自傷他害事件を減少させることが可能であろう。また、新規措置入院患者数の増減は、社会や国、県、受け皿としての病院、およびそれらを連携する医療体制によって大きな影響を受けると推測されるが、近年の新規措置入院患者数の増加には、警察官通報を主とする緊急医療システムの充実が大きくかかわっていると推測された。さらに、近年の新規措置入院患者の中には、措置症状のみならず、緊急に精神科治療を必要とする、いわゆる「精神科救急医療」の対象となるべき事例が包含されている可能性が示唆された。

During the three years from 2006 to 2008, Okamoto Hospital conducted evaluations (two evaluations) on 230 mentally handicapped persons, and the National Police Agency conducted evaluations on 58 mentally handicapped persons admitted to Okamoto Hospital, including 288 mentally handicapped persons.
