ヒト正常膀胱、及び尿流動態検査にて尿道閉塞が証明された閉塞膀胱におけるα1受容体のmRNAの発現、及びα1受容体の各サブタイプ(α1a、α1b、α1d)の各々のmRNA発現量は、平成13年度に報告した。今年度は、受容体の発現量の検討に加えて、各種α作動薬に対する排尿筋収縮反応を検討した。平成14年度に検討した症例数は、昨年と同様手術時に得られた正常膀胱10例、及び閉塞膀胱7例であった。mRNAのレベルでα1受容体の発現を検討した結果、受容体の総量は非常に少量であり、さらに各サブタイプの検討でも、発現量はごく微量であった。さらに正常膀胱と閉塞膀胱におけるα1受容体の総量、及び各サブタイプの発現比率を比較すると、その差は昨年の検討と同様非常にわずかであった。次に、採取した排尿筋から得られた筋切片を用いて、α1作動薬であるフェニレフリン、及びノルエピネフリンに対する収縮反応を検討した。その結果、正常でも閉塞膀胱でも、フェニレフリンは10^<-5>Mから収縮反応を起こしたが、10^<-4>Mで得られた最大収縮反応は、カルバコール10^<-7>M収縮の僅か5%未満であった。さらにノルエピネフリン投与では、10^<-5>Mで微小な収縮反応(1%未満)を示したが、10^<-4>Mから逆に弛緩反応に移行した。以上の結果から、(1)ヒト正常膀胱にはアドレナリン受容体はごく僅かしか発現していないこと、(2)閉塞膀胱になってもα1受容体のmRNAは、α1作動薬に対する収縮反応の乏しさが裏付けているようにほとんど増加しないことが明らかとなった。したがって、そのサブタイプであるα1dは、膀胱平滑筋レベルで刺激症状に関与するとは考えられないことが示唆された。
