昨年度より症例数を追加して再検討した。対象は1998年4月から2001年10月までに当科で診断された膀胱癌29例,腎盂癌13例,尿管癌7例の49例で,年齢は46〜86歳(中央値70歳)である.病理組織は移行上皮癌41例,移行上皮癌+扁平上皮癌3例,移行上皮癌+腺癌2例,移行上皮癌+扁平上皮癌+腺癌2例,不明1例であった.抹消血7mlからRT-PCRを施行し,血中にサイトケラチン(CK)19および20の産生細胞の存在を検出した.原発巣の病理組織においてGrade 1:5例,Grade2:38例,Grade3:10例のうち,CK19陽性がそれぞれ1例,15例,5例,CK20陽性が2例,22例,7例,pT1:2例,pT2:11例,pT3:9例,pT4:4例のうち,CK19陽性が2例,3例,3例,3例に対し,CK20陽性が2例,6例,6例,4例であった.pN-:31例,pN+:12例中CK19陽性が12例,6例に対し,CK20陽性が17例,10例であった(P=.05).採血時リンパ節転移を有していた13例中CK19陽性が4例,CK20陽性が10例で,遠隔転移を有していた3例中CK19陽性が2例,CK20陽性が3例であった.再発した16例中CK19陽性が6例,CK20陽性が15例,癌死あるいは転移巣を有して生存している15例中CK19陽性が5例,CK20陽性が12例であった.以上より,サイトケラチン20が尿路上皮癌の予後因子になりうることが示唆された.
