本研究は、医療機関を利用している日本人中高年者の医療ケア上の自己決定の認識と行動の実態およびその影響要因を明らかにし、患者の自己決定を有効に活用した医療サービスを提言する事を目的として実施した。1)本調査の結果と今後の課題本調査は平成13年度5月〜10月にかけて、倫理的手続きを経て、調査協力の承諾を得た30歳以上76歳未満の外来患者302名を対象に自記式質問調査を実施した。調査票は、意思決定に対する個人の認識を査定する尺度としてAPI (Autonomy Preference Index)を織り込み、自己決定行動の程度の実態との関連や、人口学的変数、健康状態、HLC (Health Locus of Control)等との関連を検討した。その結果、APIを構成する意思決定希求と情報希求の程度の間には有意な負の相関があることが明らかになった。また、意思決定希求とHLCの間にも統計上有意な相関が認められ、意思決定希求が高い集団ほど、external(内的自己統制力が低い)傾向がみられ、健康な人を対象に行われたHLCに関する先行研究から示唆されていた仮説とは異なる結果を示した。本調査では、自己決定行動との統計的関連はなく、年齢、性別などの変数による差もなかった。自己決定行動は、API、HLC、年齢や教育年数等の変数との関連は明らかにならなかったが、性別による有意差がみられ、治療上の決定行動においては男性よりも女性の方が、療養生活上の決定行動においては、女性よりも男性の方が、家族に依存する傾向があることが示唆された。今後の研究課題としては、変数間の関連をさらに分析し、意思決定の認識と行動との関連を詳細に検討する。また、先行研究との本研究の比較を行い、日本人の行動様式の特徴を明らかにするよう研究方法の検討を予定している。
