项目摘要

ヒヤリ・ハットや医療事故データの報告もれの検証方法を開発するために、医療事故に関する文献収集を行い、調査方法、発生率の把握を試みた。しかし、医療事故に関する定義や概念枠組みが確立されていないため、発生率の比較は困難であった。そこで、看護事故(nursing error)に焦点をあて、看護事故の分類方法を構築した。看護事故を事故発生の次元と事故分類の次元に分類した。事故発生の次元は以下の3つから構成される:I)看護ケア基準からの逸脱、II)因果関係が明らかadverse outcome、III)基準以下のケアで発生しやすいadverse outcome。因果関係が明らかな、adverse outcomeは、事故分類の次元として以下の5つに分類した:1)医療者の事故の認識レベル(ヒヤリ・ハット、インシデントなど)、2)事故の原因(知識不足、多忙など)、3)事故の発生場所(手術室、ICU,診療所など)、4)事故の種類(誤薬、記録、医療機器の操作ミスなど)、5)患者への障害の程度(障害無しから死亡まで)。基準以下のケアで発生しやすい事故としては、褥創、術後の尿路感染や肺炎などが含まれる。これらの看護事故の研究方法は、事故の分類方法とも一致していた。Iのケア基準からの逸脱は、観察による前向き研究、IIのadverse outcomeは、主に後ろ向きのデータ収集、IIIは、退院時データベースを用いた生態学的研究である。報告もれの一つの指標として、多施設のデータを比較する院内感染のサーベイランスで、2標準偏差、、または10パーセンタイル以下は、感染の把握ができていない可能性を示唆する。看護事故の多施設でのデータ収集は困難であるが、多様な方法を用いて事故の発生を把握する必要がある。本研究の成果は国内外の学会で発表した。
