本年度は不適切なカテーテルケアと細菌尿との関係を調査するため、O大学医学部付属病院で3ヶ月間前向きにデータを収集した。計117名の患者の尿を週3回培養し、カテーテルケアの基準逸脱を観察した。平均年齢は60歳で、男性が54%をしめていた。病棟は、救命救急病棟が9割近くを占めており、他病棟は脳外科と神経内科であった。約半数の患者は気管内挿管をしており、殆どの患者がオムツを使用していた。カテーテル留置期間は平均が8日、中央値が5日であった。カテーテル関連尿路感染は17名に発生し、発生率はカテーテル留置期間1000日に対し、22であった。病原体はグラム染色では、グラム陽性球菌とグラム陰性桿菌が同じ割合であった。感染者17名のうち10名が抗菌剤が投与されていた。抗菌剤投与を受けていない患者は、全員がカテーテル関連尿路感染を発症したが、投与群の発生率は15/1000デバイスデイズで、有意な感染率の相違がみられた。尿路感染のリスク因子として、高血糖、低栄養、高齢、女性、尿量低下が示唆されたが、標本数が少ないため多変量解析によるリスク調整ができなかった。不適切なカテーテルケアの観察は、バッグと床の接触、カテーテルの屈曲、バッグが膀胱より高い位置、カテーテルの固定などを観察した。しかし、抗生剤を投与されていない患者は少なく、これらのケアと細菌尿との発生を分析できなかった。このため、実験研究として、バッグの排尿、口の汚染、使用しているゴム手袋の汚染による逆行性感染の実験を行うことにした。現在実験中である。臨床疫学調査結果は第22回日本環境感染学会で発表し、また本年度のInternational Council of Nursingでも発表予定である。
