本年度は、(1)救急看護と関連のある155の看護介入について、個人属性別(年令別群、臨床経験年数別群、救急看護経験年数別群)及び所属機関の救急形態別(全次群、1・2次群、3次群、病床数別群)にクラスカル・ウォリス検定もしくはウイルコクソン順位和検定を用いて実施頻度の違いについて検証し、(2)所属機関の救急形態別にパスモデルを作成し、看護介入の実施に影響を及ぼす要因について検討を行った。結果、(1)個人属性別では年令別群に、救急形態別では、全次機関群/1・2次機関群間及び3次機関群/1・2次機関群間で155の看護介入の実施頻度に多くの有意差が認められた。(2)155の看護介入をNICのレベル2(類)ごとにまとめた上で、救急形態別に因子分析を行ったところ、1・2次機関で3因子、3次機関で4因子、全次機関で5因子が抽出された。これら抽出された因子及び、個人属性(年齢、臨床経験年数、救急看護経験年数)、救急活動状況(病床数、年間救急車搬入件数、年間救急患者数)を潜在変数に設定し、救急形態別にパスモデルを作成し統計学的に検証した。1・2次機関では、救急活動状況がActiveな施設ほど経験の浅い看護師が多く、看護師個々の看護介入の実施頻度は経験が高くなるほど減る傾向にあるが、救急活動状況の影響を受けると、看護師個々の看護介入の実施がむしろ増える傾向にあった。3次機関では、救急活動状況のActiveな施設ほど経験の長い看護師を多く抱えており、看護師個々の看護介入の実施はさらに減る傾向にあり、救急活動状況の影響を受けると、看護師個々の看護介入の実施はさらに減る傾向にあった。全次機関では、救急活動状況のActiveは施設ほど経験の浅い看護師が多く、看護師個々の看護介入は経験が高くなるほど実施頻度が増える傾向にあるが、救急活動状況の影響を受けると、看護師個々の看護介入の実施は減る傾向にあった。
This year,(1) emergency care related to 155 care intervention, personal attributes (Age group, clinical years group, emergency care years group) and emergency mode of affiliated organization (All sub-groups, 1st and 2nd sub-groups, 3rd sub-groups, number of beds),(2) Emergency mode of the affiliated organization, implementation impact of nursing intervention, and key factors in the implementation of inspection.
