项目摘要

逆流性食道炎に続発して形成され、高い発癌ポテンシャルを有するパレット食道の形成機序を明らかとすることを目的に、食道粘膜の線維芽細胞の役割について注目し研究を行った。仮説は食道の扁平上皮が逆流胃酸で消化され逆流性食道炎が形成されると間質の線維芽細胞に胃酸が直接に接触するようになり、線維芽細胞が増殖因子やサイトカインの産生をして、この増殖因子やサイトカインが扁平上皮の杯細胞に作用して分化異常をおこし腸型の円柱上皮化生をおこすとするものである。本年度はヒトの胎児由来の比較的未分化な培養線維芽細胞を用いて、この線維芽細胞に酸性の負荷を行った。pH5.0の負荷を20分間行い、その後中性pHにもどし直後から24時間後まで1時間おきに培養線維芽細胞よりメッセンジャーRNAを得た。このRNAを用いてc-fosとIL-8のメッセンジャーRNAの著明な発現充進のあることが明らかとなった。また、IL-8の発現亢進は蛋白レベルでも確認することが可能であった。そこで酸負荷前と酸負荷後のメッセンジャーRNAより、cDNAを作製し、これを用いてILLUMINA社のIlumina Bead Array DNAマイクロアレイを用いて酸負荷によって発現が3倍以上に亢進するものについて検討を行った。全体として約1万種の中で約500種のcDNAの3倍以上の発現増加を認めた。この中でPDGF-beta polypeptide, PDGF-alpha polypeptide, HBEGF-like growth factorについては、上皮系細胞の増殖と分化に関与している可能性が報告されている
