项目摘要

本研究の目的は、遺伝的課題を有する人の健康観、障害観、家族観形成過程に潜む「遺伝」に関する主観的体験を個人の生活世界に焦点を当てて明らかにすることを通して、遺伝的課題を持つ人々への看護学的アプローチを示すことである。研究協力者は遺伝性疾患または先天異常の患者、保因者または家族で、本研究の趣旨に同意した人とした。実際は、DRPLA(歯状核赤核淡蒼球ルイ体萎縮症);常染色体優性遺伝病患者の妻であり母親であるAさん、網膜色素変性症;常染色体優性遺伝病で全盲であるBさん、Bさんの娘で同疾患で片側が義眼であるCさん、Perrult症候群;常染色体劣性遺伝病の保因者で罹患した2人の娘を持つ母親Dさん、家族性大腸がん;常染色体優性遺伝病であり既にがんを発症し、家族にもがん発症者がいるEさん、筋緊張性ジストロフィーを夫と子ども2人が罹患しているFさんとした。データ収集方法は構成型および非構成型インタビューとした。インタビュー回数は、追加面接を含めて1人4〜5回としまだ続行中である。データは研究協力者の許可が得て録音し逐語録とした。データは各対象者からの面接内容を年代順に記述した。さらに健康、病、障害、家族に関する経験を、年代やライフイベンツに添って正確に再構成し記述した。そこで語られた内容のテーマを明確にし、文脈に沿って分析した。その後、対象者の健康、病、障害、家族に関する経験のパターンの共通性を分類した。本研究では、家庭内の同世代または各世代に同一の疾患患者が居るという同質性と、家族外の社会には決して求めようとしない閉鎖性から、家族員が集約と分散を繰り返していた。しかし疾患自体の体験の中に個別性があり、その個別性を認め合うことが家族の精神的集約力に関わるという可能性が示唆された。
