臨床看護師に対して、本研究で作成した足病変を有する足模型を使用したフットケアプログラム受講後にフットケアを実施した群(強化群)と足模型未使用のプログラム受講後にフットケアを実施した群(対照群)では、「フットケア技能習得状況」に違いがあるかを検討した。また併せて、「フットケア技能習得状況」の程度がフットケア後の足状態(爪形状、胼胝、鶏眼)や「技能習得状況に対する自己評価」に影響しているかを検討した。なお、被験者には、爪白癬、胼胝、鶏眼を有する者に協力を得て、糖尿病患者を演じてもらった。フットケアプログラムに要した時間は、講義40分と技術指導30分の合計70分であった。研究評価には、質問紙尺度を作成して使用した。「フットケア技能習得状況」については、患足の固定方法、フットケア機器操作などに関して、爪切り13項目、胼胝11項目、鶏眼9項目を「かなりよくできている」〜「全くできていない」の5段階で評価した。また、フットケア後の被験者の足状態についても爪形状6項目、胼胝4項目、鶏眼3項目を上記と同様に5段階で評価した。なお、フットケア実施中はビデオ撮影を行い、研究者間で妥当な評価ができるようにした。さらに、フットケア実施者には、「技能習得状況に対する自己評価」として「フットケア技能習得状況」と同項目について「かなり自信を持ってできた」〜「自信がなかった」の5段階で回答を得た。強化群10名と対照群8名の年齢及び看護師歴に有意差はなかった。「フットケア技能習得状況」及び「足状態」については、どの項目においても強化群は対照群に比べて得点が有意に高かった。なお、「技能習得状況に対する自己評価」については、爪切りの2項目(患足の固定、清潔不潔の区別)、胼胝1項目(患足の固定)のみにおいて、強化群は対照群に比べて得点が有意に高かった。以上の結果により、足模型を使用したほうがフットケア技能習得状況がよく、また爪形状、胼胝、鶏眼に関する処置が適切に行えると示唆された。今年度
