EBNに基づいた産褥期の保健指導への活用を目指し、前腹壁の代表的な筋である腹直筋の表面筋活動電位を経時的に測定し、対象者の特性との関連を分析した。測定には表面筋電計(MEB-9102、日本光電、東京)を用いて、妊娠初期、産褥1日目から5日目の各日、産褥2週間、1か月、2か月および3か月で行い、対象者29名について実施出来た。対象者には10秒間に1回のペースで3回アプドミナルカールの動作をしてもらい、サンプリング周波数10Hz〜10kHz、増幅感度50μV〜200μV/divでそれぞれ表面筋電位を計測した。分析は、得られた3回の生波形を全波整流した後、台形公式を用いて200msec間の積分値を連続的に算出し、3つの最大値の平均値を求めた。なお個人差の影響を排除するため、各対象者についてそれぞれ全測定値の平均値を用いて正規化し評価した。結果について、まず産褥1か月間は腹直筋の筋力は有意に低下した状態で推移し、回復には2か月を要することが明らかとなった。次に、分娩時出血量が400ml以上であった群は産褥2週目に筋力が有意に低くなり、分娩時出血量と発揮筋力に関して負の相関(r=-0.42)が認められ、筋力の回復過程に影響を及ぼすことが明らかになった。最後に、母乳哺育を行った方が混合哺育を行うより早期に腹直筋が回復するという新たな知見も得た。したがって産褥1か月間は腹直筋に負荷が掛かる動作は可能な限り避けるべきという保健指導をはじめとして、分娩時出血量および哺育方法によっても保健指導方法を変更するためのエビデンスを得ることが出来た。骨盤周囲の三次元計測結果に関しては、腹直筋の筋力の測定結果との対比など、今後とも机上での分析を続行して結果をまとめる予定である。

As a result, the strength of the abdominal muscles in the first month of childbirth was intentionally lowered, and the strength of the abdominal muscles in the second month was restored. There was a negative correlation (r=-0.42) between the bleeding volume during labor and the muscle strength recovery process.
